What Causes Heaviness In The Forehead Post A Polyp Removal?

Question: I had a Functional endoscopic sinus surgery for polyps removal in all sinuses & turbinate reduction. It's been 12 days since surgery but I still feel constant heaviness & pain in the forehead. How long this will stay like this ? I want to get back to work soon.

Hello

Normally it takes 2 to 3 weeks for the symptoms to subside after endoscopic sinus surgery. So you may have to wait for another week

Your doctor has given you the right medications and I suggest you continue them. However I would also start my patients on a steroid nasal sprays like fluticasone propionate and saline nasal drops. Please discuss this with your doctor

I would also like you to start steam inhalation thrice a day as it will reduce the congestion.

You can start going to work in another 2 to 3 days, just try to avoid too much dusty and polluted places.

Thank you Doctor. He did prescribe Flomist nasal spray & Physiomer Isotonic saline.

Few follow up Questions:

1. Some of my friends who had Fess did not feel headache post surgery so that made me nervous if all went with me during the surgery.

2. The medicine prescribed were for 3 weeks so should I stop everything after 3 weeks ?

3. How can I further prevent re occurrence of polyps ? Any medecations, Jal neti, yoga etc ? My doctor said only 20 % patients get occurrence but I doubt. He did not prescribe any measures to keep these polyps under check.

4. Should the periodic review say after every 2 months with ENT specialist help to keep the things under control. I reside in XXXXXXX

Hello

Headache is common after fess and can last few weeks so please don't Worry.

I would normally keep my patients on steroid nasal sprays for at least six months after surgery and ask them to practice jal netti at home as this will decrease recurrence. I suggest that you can do the same

Recurrence is above 50 percent and a regular follow up once every three months is a good way to detect early recurrence and treat immediately

Your symptoms will gradually resolve!! don't worry. You can continue just the spray after 3 weeks and stop rest of the medication

Thank you Doctor. This is indeed very helpful. Few more questions & then I guess I will be good after that.

1. 6 Month continuous usage of steroid spray, is it ok because I heard prolonged usage of the same can give you rebound effect i.e congestion. Is this true ?

2. After 3 weeks , should I practice Jal neti daily or more/less frequently.

3. I really want to avoid the re occurrence of this nasty ailment. Do you think any allergy test or any special diet like gluten free, sugar free etc would help ? I want to do everything that can keep this ailment at bay.

4. I was asked to avoid perfumes/deos/talcs etc earlier. Should I continue to maintain distance from the above & do you think using air purifier at home or in car would be helpful ?

Hello

In my patients i generally give steroids following surgery for a week, but there are many patients for whom i have prescribed steroids for more than a year. Steroids do not cause rebound congestion and are very safe, so please dont worry

Try to practice jal netti everyday, believe me, its very useful

Yes you can try doing an allergy testing and avoiding the allergen, this will definitely help reduce recurrence. With respect to gluten free and sugar free diet, i have not seen many XXXXXXX patients sensitive to gluten and therefore i have never recommended it to anyone.

If you are allergic to talc/perfumes then you have to maintain distance, otherwise there is no problem. Please remember allergy remains life long and if you are allergic to perfumes then it is better to avoid them. Using air purifiers can be useful specially in polluted cities, so you can go ahead and use it.
