What Causes Hematuria?

Question: I suffered gross hematuria around 5 days back.I am a patient of recurrent UTIs since the last 3 years and have been on ciplox, nitrofurantoin and urispas since then. The organism isolated most times was E.coli. After the hematuria incident I went in for urine culture, USG, and KFT and realised that org islolated this time was citrobacter freundii for which I am on Ceftum right now. However USG showed some other cyst in my kidney for which my urologist advised contrast CT. Now I have consulted 3 Urologists but nobody suggested any concrete diagnosis. Need help. CT report is attached.

Why this much Hematuria? Why CT doesn't suggest anything? What should i do next?

Brief Answer:
Cysts over kidney cause blood in the urine.

Detailed Answer:
Blood in the urine could be because of the cysts which contain the capillaries. The definitive treatment can only be told once the cause be found. At this time there seems no cause detected. There seems no anatomical defect and this is something good about the case. It needs a large medical history and previous medical records as well to pin point cause. Having said that I give my piece of advice to you

This seems to be a complication of recurrent Urinary tract infections. The report is suggesting a possibility of infection in the kidney earlier (pyelonephritis) leading to the cysts on the surface of the kidney. The report though does not ascertain the cause it hinted of the above possibility.

E.Coli and citrobacter or any other bacteria are originally present in the urinary tract of women. I am not sure of a good medical history here. However it seems there could be a lot of pathogens or germs residing your urinary tract and vagina that could ascend to cause recurrent Urinary infection. The more frequent the intercourse the high chances of these germs ascending and causing the infection.

So it is better to take antibiotic prophylaxis after every intercourse. I suggest you to meet a UroGynecologist or Infectious disease specialist in guiding you through this antibiotic dosage. Also Cranberry juice can be of some help to you. You can have it on alternate days. Try to clean the genital organs with soap and clean water after every intercourse. Try voiding for every 4 hours even if you do not feel the fullness this may reduce the chances of growth of germs.
