You gave very short history. Kindly give answer of few questions...

- are you circumcised?
- does the lesion occur repeatedly?
- from how many months you have such lesions?
- does the lesion occur after sex only? Or any time?
- exactly which area affected?
- please describe the lesion. Is the initial lesion vesicular? Or directly erosion? Is it reddish or whitish? How much area affected?
- do you get burning, itching or pain on the lesion?
- history of sex with other women beside your wife?
- past history of such lesion or other skin disease?
- past history of systemic disease?
- history of diseases like DM, HIV, BP or other ?

Please answer above queries and you give detail history of treatment you took. And kindly upload the photos of affected part. Then I might guide you in better way for managing your disease.

_yes i am circumcised
_ yes any time it reoccurs even after using Cansesten cream regularly.
_ i have this condition over a year
_ just before the tip inside folding area
_it looks like a very shallow cut red to normal colour and disappears for several days the reoccurs
_ no burning feeling slight wounded area fealing
_not married many partners
_no history of any disease.

Most probably herpes progenitalis

Detailed Answer:
Hi,

Welcome back..

After reading history and seeing the photos, it seems that most probably you may be having herpes progenitalis. Due to unprotected sex,you might have got herpes virus type II infection..leading to herpes progenitalis. Still, you consult dermatologist personally for firm diagnosis. Blood tests for HSV antibodies might be done.Herpes progenitalis is a localised chronic and relapsing disease. Mostly relapses occur in patients with poor immunity. Rule out HIV infection by blood test.

I would like to suggest as follow..

- antiviral like acyclovir 800 mg for 5 times a day till the lesion subside.
- low dose of acyclovir 400 mg thrice a day may be taken for long time
- acyclovir cream may be applied on the lesion
- immunomodulator like levamisole 50 mg may be taken daily at night for long time
- have protected sex
- avoid multiple partners

I hope this would help you in managing your disease. Have treatment under observation of dermatologist. Any more query ..you may ask.
