Question: history of high PSA but no other problem until now, blood in urine & stone need to be removed.
Plz explain? long term outcome, treatment, etc.etc.

Elevated serum PSA is a screening tool to detect prostate cancers. Your risk for prostate cancer would depend on how high is your PSA value. The higher the PSA the higher is the risk for prostate cancer. to diagnose prostate cancer you have to undergo a prostate biopsy.

The treatment for stones in the urinary system would depend on the size of the stone and the location. For small stones (<2cm) treatment would entail shock wave therapy to pulverize the stone for easy passage of fragments in the urine system. For larger stones you may need to undergo surgery due to the larger stone burden which is not amenable to shock wave.

Stones very large, blood has been a while - what is recovery time, what to expect pre/post surgery???

PCNL will be done for large stones.

Detailed Answer:
For large stones in the kidney what we usually do is PCNL (percutaneous nephrolithotripsy). This is done under general anesthesia where you are positioned in the prone and a small incision is done at the flank area around 2-3 cm. a nephroscope is inserted and stones will be extracted under direct visualization. After the surgery expect to have a nephrostomy tube in your back for 1-3 days and will be removed if there is no bleeding. You will be able to ambulate day after the surgery and will be sent home after the tube has been removed.
