What Causes High Alkaline Phosphatase Levels When Suffering From Psoriasis?

Question: Hello I have psoriasis and was going to be put on oral meds until they found my alk phos was high. Was tested for hep a b c negative so now I have to see liver specialist Very nervous I do drink wine but social occasions I did od a bit on wine when I had a great loss. Other than that not much How afraid should I be as I am so upset. Also take meds for asthma cholesterol arthritis and clonazepam What are your views on all this Thank YOU XXXX

Dear Doctor Thank for reply Here is list of med zocor40 mg 1 daily 7 year celebrex 200 mgs as required 5 yrs Advair 250 2 x day 7 yrs ventolin 100 mcs as required 7 yrs Betaderm .1% FOR BODY AND SCALP 1 YRS also
clonazepam .5 mgs 1 daily 10 yrs Bacofen 10 mg for 2 yrs or as required Hope you can let me know how these meds effect my alk phos levels

Zocor (simvastatin} can cause the liver problem and increased enzyme.

Detailed Answer:
Dear Mam,

Welcome again and thanks for posting your details of medications.

Amongst all the medications which you have mentioned, the only medication to cause the liver problem is zocor 40 mg.

Zocor contains simvastatin which is a lipid lowering medication. Though in general it is not the main cause of increase in liver enzymes but in some case it can cause liver cirrhosis (damage to the liver tissues) and therefore the liver enzymes can get elevated.

The rest of the medications are quite safe in terms of liver side effects and therefore not appears to be the cause of raised liver enzymes.

In psoriasis we generally give Methotrexate as main medication. But before starting the drug we need to do liver profile to see the normal working of liver. In case of the liver enzymes are raised then we are not able to give any drug which itself have any impact on liver tissues. Methotrexate itself can cause mild to moderate increase in enzymes.

Therefore it is important to not to prescribe Methotrexate till you get normal value of enzymes.

I reassure you fully that as you are negative for all type so hepatitis, so making you free free from all causes of major infections.

The only cause is the lipid lowering medication simvastatin which can be managed easily. Liver is a very much regenerative tissue so if any problem had been caused by any means can be easily managed with complete recovery.

Do not be stressed by the simple thing and just relax for the same.

I am really happy to know that you are relaxed to know the simple facts regarding your simple condition.

Serum alkaline phosphatase can increase even up to three folds of normal values. It can be managed from your side by dietary measures.

In my patients with your kind of problem I suggest them to take good liver protective diet.

A diet containing lot of fruits, green vegetables and green tea helps us a lot in improving the status of the liver.

I suggest you take grapes, avocado and turmeric in your daily routine.

Walnuts and overnight soaked and peeled almonds are also very much beneficial.

I additionally suggest you to discuss with your trusting doctor regarding certain prescribed medications for liver protective action.

liver profile will come down to normal within 1-3 months of period.

Dear Mam,

Welcome again to the forum and thanks a ot for your further follow up.

Regarding your further query: Once simvastatin is stopped then the level of Alkaline phospahtase comes down in 1 month of period and within 3 months is almost near to normal level.

If I were your treating doctor then would have planned following:

- To stop simvastatin for a time being period.

- To give liver protective drugs such as Ursodesoxycholic Acid (Udiliv).

-To give a liver protective diet such as grapes, avocado, Walnuts and turmeric in your daily routine.

- To check the liver function tests including SGOT, SGPT and Serum alkaline phosphatase monthly for three months.

- To check for lipid profile. If at all lipid profile is higher then would have given lipid lowering drugs intermittently with 3 monthly blood profile to know the lipid level as well as any effect on liver.

I am pretty sure that your liver profile including Serum alkaline phosphatase will come down to normal within 1-3 months of period by all the suggestions given above. I suggest you to just relax and do not worry.
