Question : i am subba rao from XXXXXXX i was detected with high sugar with hba1c at10.5.the doctors here gave me istamet 50/1000 once daily,glizid m twicw daily,telma h once daily after continous use my hba11c is 7.5,cretinine one month back 1.78 ,urea at 58,after 4 days i tested again in another lab and cretinine was 1.4,my local doctor asked me to discontinue telma h and istamet as my bp was at 160/90 at that time and asked me to take nefrosave twice daily,amlodipin5 mg twice and glizid m twice and saxagliptin once after seve n days i got tested again from thyrocare, and cretinine was 1.04 bun at 16.7 and hba1c at7.5 as pr the doctor here he is trying to save my kidney.so if it true how many days should i continue with nefrosave.all lipid,liver profile are ok, thyroid is ok.there is no appreciable problem with my sight,i only use reading glasses as i am 64 years old.

Welcome to HCM. I have gone through the question and understand your concerns.
The initial elevation in serum creatinine could be due to uncontrolled diabetes or due to telma H used for hypertension.
Telma H is usually kidney protective and the recommended BP medicine in diabetes. But in some patients instead of improving kidney function it decrease it and cause sudden rise in creatinine. So you can discontinue it as of now and have a trial of restarting it once creatinine is normalised- so that you can confirm whether it was due to diabetes or the drug.
Regarding Nefrosave:
There is no recommendation of using this drug in renal failure. There are studies which show that this drug is beneficial in kidney disorders and in some there is no benefit. It is relatively free of side effects. So you can continue it - probably for a period of 3 months more after your creatinine is normal. Again this is doctor preferences - no guideline recommendations.
