What Causes High Eosinophil Count?

default
Posted on Wed, 20 Sep 2017
Question: What can be the cause of eosinophilia. Is it due to some allergic reaction or some kind of (parasitic or other) infection.
I have eosinophilia for quit some time, with relative range from 10% - 18%. I was initially prescribed Diethylcarbamazine (Hetrazan/Eofil) but that did not reduce the count. Then I was prescribed Deflazacort along with Hetrazan and that did bring my count to normal. However after the course was stopped I got some cough and cold and my count again increased to 11%.
In general I don't have any daily symptoms but just things like after swimming I get nasal congestion and sometimes I get cough and cold with eosinophilia (with some nasal and chest congestion).

So what could be the reason for my condition and do I need to follow current medication (Hetrazan plus Deflazacort) and any further investigation that should go for?

Brief Answer:
Mild eosonophilia

Detailed Answer:
Hi
Welcome to healthcare Magic and thanks for putting up the query.
You have mild eosinophilia. There are many reasons for this like parasitic infections or allergic disorder or any blood disorder or it can be idiopathic.
You should get an absolute eosinophilic count aand peripheral blood film done and a stool examination for parasites done.
Continue with hetrazan as advised by your doctor.
Get a xray paranasal sinuses done.
Follow up with the reports so that i can suggest you better.
Take care
Thanks and regards

Hi,
Firstly I have attached additional reports (blood, stool, xray) for your reference.
Regarding information you asked for
1. The absolute eosinophilic count it is 930 / micro lit.
2. The stool report is normal wrt parasite and ova (none are detected)
3. The xray of PNS impression is Allergic Rhinitis

So based on your response I understand that there can be many causes for eosinophilia. Looking at my report it seems that probable cause is some allergic disorder or perhaps idiopathic. So I have following followup questions for you and would help me a lot in evaluating my situation and how to deal with it.

1. So what in your opinion seems to be the cause of my eosinophilia?

2. If cause is allergic then in this case does hetrazan helps. Because as I read from various sources that hetrazan mainly helps in case parasitic infections only. Also earlier when I was only on the course eofil/hetrazan it did not lower my eosinophilic count. It was only after taking cortimax (Deflazacort) it helped with my eosinophilia.

3. As mentioned I am also taking cortimax, should I continue with it along with Hetrazan or should I discontinue Hetrazan and only continue with cortimax?

4. Doctor has also prescribed Ennhale (Fluticasone) nasal spary. Is it OK to take it for a long duration say a month?

5. Does antihistamines help in my case or prescription of cortimax is better.

6. Any idea what could be causing this allergic reaction. Is swimming to be blamed for my condition and should I continue with swimming or need to stop it completely.

I would be grateful to you for your responses to my queries.

Thanks XXXXXXX

Brief Answer:
Eosinophilia

Detailed Answer:
Hi
Thanks for the information. I have seen the reports.
The cause for eosinophilia in your case is probably allergic.
The stool test being negative might be a result of hetrazan intake. As the stool test is negative you can discontinue with hetrazan. A long term intake of steroids can lead to side effects. My advice is you should taper down deflezacort and than stop.
Topical nasal steroids are very safe. You can continue to take with doses adjusted.
Antihistamines will help if you have nasal symptoms. As per the x-ray you have a deviated nasal septum with turbinate hypertrophy. You should take oral decongestants for few days.
You can continue with swimming once nasal symptoms subside.
If nasal symptoms are severe and recurrent, surgery can be done to correct nasal pathology.
Any further questions are always welcome.
Thanks and regards

Hi,
Thanks for the suggestions. I myself don't like to continue steroids for long and I have a taper down dose of deflezacort and that will stop in 5 days,

I just had one question regarding hetrazan.
Looks like my condition seems to be allergic and in this regard I would like to share some background.

Firstly stools tests were negative for parasites even before starting hetrazan. (Note I had eosinophilia that time).
Then I started course of hetrazan only and eosinophilia did not come down. Also stool tests were always negative for parasites.

It was only after deflezacort was introduced along with hetrazan that my count came down.

So question is does hetrazan helps with treating eosinophilia if underlying cause is not any parasitic infection but only allergic, like in my case?

Thanks XXXXXXX

Brief Answer:
Eosinophilia

Detailed Answer:
Hi
The steroids have an anti inflammatory action and this leads to decrease I eosinophil count but that is not the treatment.
Yes hetrazan is known to help in eosinophilia without any parasitic infections. It is documented but still under study.
