What Causes High Fasting Sugar Levels?

Question: Hello Dr. Shankdhar,
Greetings.
I am 59 years old Male and I have been diabetic for the past 20 years. When I got this problem 20 years back, it started with very high blood glucose levels (450+ fasting) and near 500 post lunch and I was hospitalized that time 20 years back and I was on insulin and strict diet control and exercise. After one or months, the numbers have come down drastically and I was changed to tablets from insulin. From then on I have been very disciplined on the food habits and also exercise. I walk around 6 kilo meters in one hour in the morning every day.
I perform yearly health checkups of all the organs including eye test, cholestrol test, kidney related tests, ECG, etc. and they have been under control.
Now in the recent past, 2 years back, I switched over to homeopathy and I was taking homeopathy medicine also along with some of the allopathy as well. But the sugar was not under control that time and it was showing 200+ fasting and 220+ post lunch.
Hence, I have consulted a diabetologist about 6 months back and I have been following his advise since then. Now the sugar levels are under control and I perform tests once in a month.
Last test was done about a week back and following are the results.
Fasting: 134, post lunch: 133
Creatinine: 1.4. Last month it was 1.3.
Albumin: 2 Plus last week which was 1 plus last month which has come from complete urine examination.
I was also noticed thyroid 2 months back and it was 14. After medication last week, it was showing 1.3. Following is my present medication schedule.
Early morning on fasting: Thyronorm 50 mcg
20 minutes before breakfast: Volix Trio Forte 2 - 1 tablet
After Breakfast: Albunil (I have been advised to take this from last week)
Febustat 40
1 tablet each
After lunch: Rencap & Calcimax - 1 tablet each
20 minutes before dinner: Volix Trio Forte 2 - tablet
After Dinner: Albunil , Urimax 0.4 mg, Ecosporin 75/20 - 1 tablet each

My present concern is on the creatinine and albumin levels and also the glucose fasting levels.
As I mentioned, I follow very strict diet discipline and also walking for 6 KM every day in the morning for one hour.
Am I at risk now and if so, how can I further work towards reducing the risk?
Is there any natural way I can reduce risk
Please advise

Thanks for the query.
I have read your query and understand your concern.
Basically speaking you seem to be settled except that you have slightly high fasting sugar level and presence of albumin in urine.
Your serum creatinine levels seem to be ok and would need a close monitoring.
Get your 24 hour urine estimation of urinary albumin.
A level below 30 mg is all right but if it is between 30-300,then it is red flag and means kidneys are not filtering protein properly and this is called microalbuminaria and is indicative of early kidney damage and level above 300 is indicative of advanced kidney damage.
What are the natural ways to bring down urinary albumin and serum creatine are..
Quit smoking and alcohol
Regular exercises.
Immaculate control of thyroid functions,blood pressure and blood sugar levels.
Make sure to keep your blood pressure below 120/80. May add an ACE inhibitor like RAMIPRIL to your treatment after discussing with your doctor.
Take Low intake of protein,fat,sodium and sugar.
Drink plenty of water.
Drink herbal and green tea

You may add some insulin in night like lantus to take care of your fasting sugar in consultation with your doctor.
It would be better to consult a nephrologist and get your kidneys evaluated properly.
If nephrologist finds that your kidneys are compromised,then it would be better to switch to injectable insulin instead of oral anti diabetic drugs.
