Question: Hi Doctor, I want to discuss with you about family my infection case.
on Friday my daughter who is 7 months started to have a fever so we started giving her paracetamol syrup whenever the temperature is 38.5. On Saturday just around the afternoon she developed fever again which was about 38 degrees, so I told my wife to wait until the temperature is 38.5 then give her the paracetamol in just 15 minutes from that my daughter got febrile seizure which was the scariest thing that ever happened to us, she was only looking up and not responding to any stimuli, I immediately jumped in the car and took her to the emergency room, luckily we had a calm doctor who just suggested us to reduce the fever by giving her bath and she gave her ibuprofen syrup 4ml, she got better and she discharged my daughter with adol 125mg suppository every 4 hours for 24 hours and PRN after the 24 hours. she is now doing well. she said it seems like a viral infection so no need to give antibiotics

The other side of the story is that also me, my wife and my son (who is 2 years old) developed high-grade fever, my son started on the same night I took my daughter to the hospital, my wife had got it on Sunday and I got it on Monday all this week.

I took my son to Hospital and the doctor said it's probably a viral infection after looking at his throat. he also prescribed him adol suppository 125mg every 4 hours, a dosage of one and half suppository. the problem is that he doesn't like the suppository and he cries so loud so we only give him ibuprofen and paracetamol syrups alternatively. t

my wife had a fever with vomiting and diarrhoea on the next day and since she was breastfeeding i decided to take her to emergency to help her with iv fluids, at the emergency they gave her IV fluids, metoclopramide, and perfalgan, and they discharged her with metoclopramide tablets only.

I have a severe headache that comes with fever of 39.2 with neck pain and joints pain. some times only paracetamol 1000mg doesn't help me so i take both paracetamol 1000mg with ibuprofen 600mg together.

my questions.

1.what do you think of this case, is it viral or bacterial? both doctors for my son and daughter decided to not give them antibiotics and they thought it's viral, my wife doctor gave her Suprax antibiotic just to be on the safe side but I decided to wait with it as she is better now with the vomiting and diarrhoea

2. My son doesn't eat anything at all, only a few meals in the afternoon, in the morning when he wakes up I have to give him ibuprofen 7ml because of his fever, but I give him on empty stomach. I read that ibuprofen is dangerous on an empty stomach, is it safe to give my son ibuprofen syrups even though he has an empty stomach? sometimes at night while he is sleeping i give Voltaren suppository

3.do you think we should start with antibiotics.

4. Today i have weird sensation on my throat that i feel goes all the way down to my chest, when i take in a deep breath either from my mouth or from my nose, i get a cold air feeling coming in my throat all the way down to my chest, and it makes it difficult for me to breath, what's this symptom caused by?

5. is it safe to take paracetamol 1000mg with ibuprofen at the same time for me? I take it because the fever reaches to 39.3 and doesn't go down with only one of the medications.

6.what do you suggest us to do.

I have gone carefully through the details and understand your concern. Let me try to address the issues one by one.

1. Most often such symptoms are due to viral infections and antibiotics are ineffective in such cases. I would not advocate giving unnecessary antibiotics. It does not make the child additionally safe.

2. Ibuprofen is most ill reputed for causing gastric irritation and gastritis and therefore should not be given in empty stomach. However, if given in full stomach it is hardly a cause for concern. Rather than voltaren I would advocate paracetamol suppository. It is much safer.

3. No I do not think that you should start antibiotics unless blood tests indicate so.

4. Your symptoms too seem to be caused due to a viral infection (possibly the same one). I would insist you to take some antiallergic like cetirizine and also steam inhalation.

5. It is safe to take paracetamol. 500 mg is the fever reducing dose whereas 1000 mg is the pain reducing dose. I do not think that you need to take ibuprofen additionally. As I have already mentioned, ibuprofen is known to cause gastritis.

6. Observe the child for lethargy. If that is not present no need to worry.

Hi Doctor, thanks for the response.

this morning I had difficulty breathing, every time I take a deep breath I feel pain in my throat that I feel goes to my chest and I cough, nothing on exhaling just when in inhaling. my wife has also the same thing but not as bad as I felt today. I went to emergency today, my o2 saturation was 91 the nurse asked me to take deep breath couple of times, which when I did it went up to 95 to 97 when however with every deep breath that I took I felt the cold pain on my throat to my chest with chest tightness and coughed every time I took a deep breath . the doctor checked on my chest and said it's clear no abnormal sounds, he also checked x-ray which was normal, he gave me Ventolin and Pulmicort which didn't really help I still have the symptom and o2 saturation was still the same.
The doctor did CBC with WBC of 12000 and CRP was 180, he told me that my CRP is extremely high and he is suspecting influenza virus, however, he said still take levofloxacin to be on the safe side.

1.what do you think I should do about the above situation I still have the pain when I take a deep breath. should I take levofloxacin antibiotic or?
2.what causes this very high crp of 180 (the normal range was from 0-6)

3. my wife also has the same breathing problem with teeth pain and nausea feeling and body ache for two days, I read online that these symptoms could be due to heart attack, is that true?

4. my sons fever is better now, last time we gave him Voltaren suppository was about 12 hours ago and it didn't come back, until now. however, he vomited at night. what is the cause of the vomiting?! is it the GI upset that you told me about the earlier answer? what can we do about his vomiting, he vomited only once at night while he was sleeping on the bed.

1. & 2. The raised WBC count as well as the raised CRP point to a bacterial infection and so treatment with levofloxacin is a good idea. Unless you have wheezing and respiratory distress, Ventrolin or Pulmocort are unlikely to help. Steam inhalation is quite essential.

3. The mentioned symptoms do not seem to point to heart attack. However, it would be a good idea to visit a doctor and to get her evaluated.

4. The vomiting could have been precipitated by the gastric irritation caused due to ibuprofen. If the vomiting persists it would be a good idea to give some antiemetic like ondansetron. Avoiding ibuprofen should otherwise be enough to resolve the problem.

Hi Doctor, thank you so much for your clear responses. My son vomited again on the bed while he was sleeping just like before.

Doctor my son didn't eat anything all day, except some fruits in the morning and some juice in the evening, do you think this vomiting can cause him dehydration. Should I take him to emergency for fluid IV?
Or should I wait till I see on him signs of dehydration? What are the signs of dehydration in 2 years old child ? He looks ok he accepted to take one zip of water but he said he feels like he is falling but he responds and laughs normally no more fever

If the child can take oral fluids (ORS) you need not go for IV fluids (which have almost similar composition). If the intake of water and salts can balance or exceed the amount of fluid vomited out, and if the urine is ok there is no reason to worry and dehydration will not occur. If the frequency of passing urine comes down dehydration would need to be excluded and you would need to take the child to a hospital.

Doctor, he didn't drink any fluid after he vomited tonight, he had just one sip of water and one sip of juice and after that he just refused. he was so tired and sleepy so, I just put him back to sleep.

I observed him, he seemed very sleepy while he was talking, I'm not sure if that's lethargy? or if it's just because he woke up in the middle of his sleep vomiting and he just wanted to go back to sleep?

he had some tears in his eyes when he cried, he didn't seem to have sunken eyes or showing any obvious or clear signs of dehydration, his lips didn't look too dry.

I will get him ondansetron as per your suggestion so that I can give him tomorrow morning, maybe he doesn't want to drink or eat because he is nauseated.
could you please suggest me the dosage of the medicine, he is 12.5 kilos 2years and 7 months old child.

Thanks for writing back. Sorry for the inadvertent delay. Having tears in eyes does not point to dehydration. I would not really worry about the sleep. Monitor the urine frequency. If it is reduced, increase the fluid intake or visit a hospital. If it is fine, no need to worry.
