What Causes Hyper-religiosity, Poor Social Interaction, Compulsive Decluttering And Aggressive Behaviour?

Question: Hi,
My cousin has been suffering from schizophrenia since 8 years and had been on treatment.
But one day she suddenly started worshiping God in the night and did not go to sleep and continued whole night. When we tried to control her, she became aggressive. She locked herself in the room and started throwing away all her belongings from the window - such as clothes, her favorite watch, mp3 player and even the money from her purse.
She stopped eating food and was drinking only water. This continued for a week. We consulted the doctor who then prescribed some sort of tranquiliser injection. After which we admitted her in the hospital. After 1 week's treatment we brought her back to home.
Since then she has been on regular medication. But from that day onward and till today, though there has been good improvement in her activities, she never stops chanting god's name. She does all her daily activities on time by herself, washes her clothes, watches TV, takes food on time, takes tea and fruits on time, and sleeps on time. But though she is doing all these activities, she does not stop chanting. That chanting continues through out the day until she goes to sleep. The chanting is loud and continous.
We had taken her to the doctor, but during those 15 min at the doctor's clinic she controls herself and stops chanting. Once we are driving back to home, she starts chanting again.
She stays at home only and does not come out or meet any people, does not come to theater for any movie or any restaurant. She just stays at home chanting and doing her daily chores.
She is 30 years old now. We are worried what is the problem with her. Is it psychosis or schizophrenia or OCD? Why she does not stop chanting and live normal life like others?

Please help.

I can understand your concern but I need more details of her symptoms to comment on her probable diagnosis. Please provide me more details if possible-

- Like when her symptoms started? How they progressed? What were her main symptoms at time of her first consultation to psychiatrist?

- Did her symptoms presents continuously since last 8 years or she is showing symptoms in episodes?

- Any relief with medicines? What are her present medicines? Any refusal to visit psychiatrist or any refusal to take medicines?

- What are her presenting symptoms apart from chanting religious things?

- How is her personal life, educational progress? Is she doing any job or staying at home only?

If we consider her present symptoms like over religiosity, poor social interaction, breaking her things and becoming violent on controlling her etc then these symptoms point towards some psychotic episode or probable mania of bipolar disorder, but more details are needed to make diagnosis.

1. The first time we consulted psychiatrist was when she was in college and she started saying that others were talking bad about her, had suspicion etc, they wanted to harm her and intentionally not allowing her to progress in studies etc
she used to complain daily. Doctor told that these are hallucinations.

2. She has been on medication all through out these years.

3. Currently she has been prescribed clopixol depot injection (every 2 weeks), daily tablets - quitiwell, amisant. She takes her medicines regularly by herself.

4. No other issue we see. Only chanting daily. And staying at home.

5. She was very good in her academics in school and college. Used to be topper.
She was campus selected and even worked in MNC for 2 years.
But again she used to start complaining about co-workers, boss, collegues etc. So finally she quit. She stays at home now.

6. Throwing away things happened only once. Now she is very calm, does not bother about others, just watches TV and does her chores.
But all these while chanting loud.

Her symptoms suggest Delusions seen in Schizophrenia

Hello thanks for providing details

The symptoms that people are talking about her, suspiciousness etc were not hallucinations but Delusions of Reference and Persecutory Delusions. These symptoms are seen commonly in psychotic disorders especially Paranoid Schizophrenia. Her illness started when she was 22 years old and this also points towards schizophrenia. Her illness is continuous which also points towards schizophrenia.

In Paranoid Schizophrenia patients respond to treatment but the illness runs a long course. Individuals can function normally if they are given regular proper antipsychotic treatment.

Her current symptoms of chanting could be occurring due to either the psychotic process itself or due to obsessive ideas in her mind. Obsessions are also seen commonly in patients of Schizophrenia. To avoid some unwanted event or thought she might be chanting loudly. With treatment of antipsychotics her symptoms will decrease in about 1-2 months. No need to worry.

But you have to provide her medicines for long time. Even some patients need life long treatment. She has relatively preserved personality so proper treatment will not worsen the situation.
