What Causes Increased Pain And Pressure In The Chest?

Question: Hi I am A 48 yr old female. I started with chest pressure last night. Assumed acid reflux. This morning the pain is still there. Pain under my right shoulder blade and up the right side of my neck to my ear. No increased HR Or clamminess. The chest pain feels like something is stuck in my throat.

Have gone through your details and i appreciate your concern.

Going through your history there can be two possibilities-
Firstly it can be Angina-Angina is a symptom of coronary artery disease.
Angina feels like squeezing, pressure, heaviness, tightness or pain in the chest. It can be sudden or recur over time.Radiation of pain to back and shoulder is highly characterstic of angina.Though gastritis ,Esophageal reflux and Heart burn can also present with same symptoms.
However as in your case pain is not subsiding from last night i would suggest you to go to the nearest emergency room and get a EKG done.Cardiac troponin are present in blood and are very sensitive to rule out myocardial infarction and unstable angina.
A 2d echo should be done to rule out regional motion wall abnormality.

Once cardiac investigations are normal.
Treatment and investigations should be done to rule out GERD and Gastritis.
An upper g.i endoscopy can be planned by your Gastroenterologist.

Till the time you reach Emergency suggestions are-
Don't panic.
Lie down flat on your bed.
If you have a blood pressure machine record your blood pressure,If systolic blood pressure is more then 110mm of hg take a single dose isorbide dinitrate sublingually and lie down for 5 minutes.If pain still not relieved go to emergency room.
