Question : Hello Doctor,
Wanted to update you that off late the pain in scar has increased. No improvement from the last 1 month except earlier I used to feel tightness along outside and inside the anus. And now I feel tightness just outside the anus. But the pain and soreness has increased. Is it the healing process going on because now I am start losing hope that I will get better ever again. Please suggest.
Hexilac cream is increasing pain and soreness. Lox gel is the best option to decrease the pain.
I spoke to my brother in law who is a maxillofacial surgeon about Deflazacort but he is not sure if it will work or not. Please share your opinion.
He is also exploring the option of triamcinolone injection. Please share your opinion.
Lastly please share your opinion what you would suggest at this point. Should I wait or go ahead with any of the mentioned treatment. My surgery was done in May 2016 and scar was formed in August 2016.
I had also got the MRI done and the report of it is perfectly ok.
Please suggest as I am losing hope now.

Hi.
Please do not loose hope is the first thing I would like to say.
Due to the positioning of the scar, we need to think twice before any drastic step.
Let us consider the following:
Surgery was done in May 2016 and scar formed in August 2016 - 10 months since surgery and about say 7 months since scar formed.
Pain and soreness in the scar indicate that there is a possibility that it is going into the stage of keloid and instead of helping the Hexilac pain is increasing the pain and soreness. This means you have to stop this at the moment.
Since Lox gel is helping you, please continue Lox alone and stop applying Hexilac.

You can certainly try Deflazacort for 9 days tapering the dosages every three days. See the result and then only consider local injection of Triamcinolone if Deflazort fails to give at least some response.

Use doughnut pillow to sit on in office and after work, in house. This will reduced the strain on the scar in sitting position; you can get this online if you search the net.
Use mild laxative if there is straining at passing stool.
Do not force the stool through as it will directly affect the scar.
I do advise a combination of equal amount if honey and Vaseline to be applied on the scar but never tried near anus, soothes the scar very well.
Continue Lycopene/Lycored.

Hello doctor,
Please suggest the exact dosage of Deflazacort to be taken and how it is to be taken.
Can i start applying Manuka Honey onthe scar and see if there is any improvement.

What is your weight?
What is Manuka Honey? Is it a trade name or?
Equal amount of Vaseline and honey may help you.
You need a valid prescription for Deflazaort- the dose for first day is 0.5 mg/kg for first three days and is tapered over every 3-4 days.

My weight is 68 kgs.

Please provide the detailed course of Deflazacort with all the details and how to take it.

Manuka honey is the purest form of honey with more than 90% pollen count from new zealand. Can i apply it on anal area?

Nice to know about Manuka honey. You need to mix a few drops with same quantity of Vaseline and apply - rub it very lightly.
Please do this for a week and let me know the results.
I hope this not only suits you but helps to soothe the scar and stop the pain.
If this fails, then only will consider steroids.
Please do not apply steroid creams locally.
