Question: Got my mother operated for carbuncle yesterday 10 IST. Her sugar ranging between 250-340 mg/dl even after 3 insuling of 10-12 units. Diet as per norms for diabetic patient. BP - normal but after knowing high sugar 169/93. Antibiotic cefoxime as per culture & sensitivity report. Still has not passed stool? Any issue with pancreas or its failure looking at sugar values or anyother issue?

May be she also needs to be started on a insulin sensitizer. You need to consult your treating physician for this. Also sometimes, presence if pus in the body make sugar levels very difficult to control. Now that the carbuncle has been taken care of, the sugar levels should respond in a favorable direction. Its not pancreatic failure, rather is mostly sort of insulin resistance. About the constipation, as before the surgery she must had been kept fasting and might had not eaten much post surgery, so that is nothing to be worried about. Thanks for the reply. From morning today her dosage changed to Human Mixtard 12-0-8 before eating and glycomet gp1 1*1 after eating. Hope this dosage is ok? She was able to pass normal stool in the morning. Also her bp was 160/100 as she got aware of her bp. Does bp increase cause any effect? Rosugalin 1*1 is dosage. Hope it is ok.?

Brief Answer: it will take some time. Detailed Answer: Hi XXXXXXX The dose of insulin and antihypertensive drugs are never fixed, they are changed regularly depending upon the response. Dont worry about them, the treating doctors will take care of that. Increased blood pressure is harmful to the heart and blood vessels. That needs to be controlled, but it will take time to come down as abrupt change is also harmful. Wait and see for a few days more. Everything will be fine only, dont worry. Thanks for the previous reply. She got discharge 2 days back and looks quite fresh. We are told to monitor her sugar before bfast, lunch and dinner. If it is above 180 mg/dil doctor has said to give insulin 8 units and glycomet gp1 all time after these meals. It keeps on fluctuating between 100-230. Hope this diabetes dosage is ok? We are doing regular dressing for her. Doctor suggested for skin grafting. Her one hole behind back is around 8x4 cm oval and nearly 1 cm deep. Also another hole on her right side tigh is 2 cm and 0.5 cm depth. Hope it will get healed and my mother feels that she is not ready for another surgery. What do you suggest, does the healing takes very long time by conventional dressing? I am observing her from past 2-3 days, her body slightly vibrates when she sits quietly. This happens sometimes with many people whom I have observed, Is this normal? Also when she sleeps she rubs her leg which was a problem from 8 months for which doctor suggested ezeepam which he changed to sylkam. The surgeon after removal did not give this as she was able to sleep. She also moves her shoulder, her eyelids vibrate and lips, chins move were sloVery slowly. These seems to be symptoms of twitching after surgery, will they have any effect on her.?

Brief Answer: its normal, dont worry about the twitching Detailed Answer: Hi XXXXXXX I must say you have a keen observation. Yes those are muscle twitching and are normal. Now coming to the main question, yes a wound of 8*4 cm might take months to heal by conventional dressing. Skin grafting will be a better option. It should do the job in a few weeks. Also it important that her sugat levels are maintained near the recommended levels to ensure complication free and faster healing. And the one on her thigh can either be left to heal by dressing or can even be sutured when it starts healing. Regards Dr. Ashish Verma
