Question : hello doctor

hope you are in good health.

i am writing this mail regarding my father's mental health.

symptoms-
1. he has started speaking a lot after drinking alcohol.
2. he doesn't sleep easily at night, and wakes up early.
3. he has stop eating food. he just eat once a day or once in two days.
4. he has become very aggresive.
5. he looses his control after drinking alcohol.
6. he has started domestic violence a lot.
7. he has disrespected all the relatives that everyone has left him, even his parents.
8. he doesn't accept his mistake, neither he realize, therefore blames other for their mistake.
9. he is very much optimistic about future.
10. he has started worshiping lord XXXXXXX a lot, in such an intensity that he constantly listens to his bhajans.
11. he now believes that it is just the god who is with him and he is doing all this so that he can walk on the path of spirituality.
12. he has lost weight to 45kgs.
13. in the past few months we have spent almost all our savings.
14. he now doesn't fear from taking risks.
15. he thinks that everyone think of him as wrong.
16. he is very much aware when aggression and ego triggers him and about his physical conditions.
17. he avoids important meetings sometimes when he doesn't feel like attending it.

it seems that he has now only one hope to live that is his god. as everyone has left him. but it must be noticed that he is functioning normally, doing all his daily chores and meetings, everything normally. above symptoms are seen throughout the day but majorly after alcohol consumption. but i fear as i don't know till when can he work normally. i really don't want to wait till the day when he collapses himself or become bed-ridden.

these symptoms are observed since past 5-6 months. they have increased since then. resulted in loss of relationships, savings, health and control.

he drinks alcohol since past 5-6 years as a regular drinker at night. his personality is aggressive since childhood. but everything has increased. he is very intelligent since childhood.

please tell me what are the disorders he is suffering from, the severity level, the future outcomes if go untreated, and possible actions steps now.

also please tell me the MEDICINES i can give him at home, that he doesn't come to know about.

From the history provided there is 2 possible psychiatric problem currently:
-Substance induced mood disorder
-Manic episode

Although treatment is usually same for these issues. In such illness usually person do not understand nature of their problem, so family members need to take proactive and timely steps towards treatment. Such cases can not be leave untreated because of complication in personal, social and medical conditions, which you can already see in his case.

Following steps should be taken:
-Meet nearby psychiatrist (you can meet me if you are in XXXXXXX ncr) for his psychological and medical assessment.
-Medicines like antipsychotics are used in such cases. Medicines are prescription medicines and can be advised only after seeing patient. Other supportive treatment also need depending on evaluation findings.
-Better to get patient admitted if he do not take treatment or continue to take alcohol to harm his body.
