What Causes Intermittent Pain In Lower Abdomen?

default
Posted on Thu, 2 Jun 2016
Question: I have had an intermittent severe pain/cramp in my lower abdomen which comes and goes. It may happen twice a day or not at all. It is so severe that I literally can not stand up straight. Bending over at the waist or sitting lessens the pain. The pain lasts for about 15 seconds and stops. If I were to stand up straight it would get worse. It usually happens after I have been sitting for a while and then stand and start walking.

Brief Answer:
Need more information

Detailed Answer:
Hi Dear !! Thanks for your query to HCM.
Read and reviewed facts of your query in context to acuter intermittent pain in lower abdomen.
As the pain comes only after walking it needs to be worked for various causes, which would need physical examination from Surgeon/ Gynec Specialist doctors.

Before further suggestions- I would need more information as follows-
How long you are experiencing this pain?Is it with burning in urination?
Which side of lower abdomen-right or left/ or on both sides -does this pain occur on walking?
Does it occur after partying , and with bloating , nausea ?
Any sweating with this pain ?
Any more information about past illness with such a pain?

Have you investigated for such a pain anytime in past with USG/ X-ray abdomen?

As there are many causes of lower abdominal intermittent pain of very short duration, I would need more information as asked above, before I suggest further about how to deal and manage such a pain in your case.

To answer your questions:
I have no pain upon urinating
The pain is almost always on the right side but has been on the left once or twice
The pain has been bothering me for about 6 months
I have no sweating, or nausea
I do not party so I have had no issues there.

I have not investigated this before but since it seems to be happening more frequently, I am getting concerned.

The pain does stay with me on walking although it is so bad when it happens that walking is almost not a consideration. It has only happened just after rising from a seated position and attempting to walk.

The only problems I have had in the past is a hysterectomy in 1995. I was carrying a lot less weight than I am now.

Hope this helps.

Thanks Much!

Brief Answer:
Recurring SIBO with Surgical Blind Loop from post-hysterectomy adhesions!!

Detailed Answer:
Brief Ans-Recurring SIBO with Surgical Blind Loop from post-hysterectomy adhesions!!

Hi Dear !! Thanks for elucidative information in last update.
Read and reviewed it with the facts of your query.
Intermittent sever cramp like pain more on right lower abdomen- is the problem in your case.
Voltaren and tylenol will not help you so stop it.
Explanation of your lower abdominal pain-
The past history of hysterectomy with post- surgical adhesions over years, has mostly lead to the surgical blind loop syndrome with Small intestinal Bacterial Overgrowth(SIBO) from time to time.Lately the frequency of this has increased and is causing intermittent pains which would reduce on its own, when the partial adhesive obstruction would reduce on its own to relieve the pain after the crampy pain in the obstructed bowel loop.
You might have experienced borborygmi or bowel noises after this pain gets reduced, or may be this might be followed by loose motions from the SIBO bacterial diarrhea there after.

The recurrence more frequently in last 6 mths explains this cause in your case.

Do's -Suggested-
Whenever you get such pains- Get the USG abdomen and Standing X-ray abdomen- which could give a lead to the fluid and gas levels- to fix and confirm the diagnosis.
Clinical examination would find rigid abdomen / lower right abdominal tightness with painful rigid abdomen, if you examine with Surgeon-during/ or immediately after this bout of pain.

To reduce the recurrence of these pain attacks, plan for after conservative treatment with -
Avoid spicy food
Check with Surgeon and get following treatment for reducing and controlling SIBO- with Surgical blind loop Syndrome in your case.IF not treated the adhesions would increase in number and would aggravate the painful attacks from more bowel adhesions from repeated SIBO attacks.
Non-Spicy / bland diet for 2-3 weeks
Tab- Bentyl /
Tab Ciplox- Oz /
Tab-Vit B12 and Tab Folic acid-
to correct the bad effects of Surgical blind loop-from post-hysterectomy flimsy adhesions in your case.

Surgical Adhesiolysis-
If the above conservative treatment fails to reduce the frequency of right lower abdominal cramp like pain, Laparascopic adhesiolysis would give definitive relief.

Update Additional information on-
what was the reason for which you underwent Hysterectomy in 1995- 20 yrs back?Whether it was total / or ovary conserving hysterectomy ?
This would give more insight in to the cause and reasoning of your lower abdominal pain, if you need so.

Hope this reply would help you to understand the cause of the right lower abdominal pain of intriguing nature. Also this would help you to plan better treatment with the help of your doctors there.
