What Causes Intermittent Pain In Lower Left Pelvic Area?

IBS, cancer, kidney problems least likes,
Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
To recapitulate: Male/31 - Medical history of Acid reflux and Anxiety - on OTC acid reflux medication - dull achy pain in lower left pelvic area - comes and goes - sometimes with pain in the left testicle and left hip - acid reflux - smoking - drinking beers - - moderate pain in stomach area - feels gas on pressure in the area you described - dull ache left lower back with pain in the left testicle and hip - nausea sometimes ...
- Wants to know if this is serious - ? IBS - wants detailed feedback - can it be caner or kidney problem -
My thoughts as per the detailed history you have provided:
- Pain in the testicle, hip and low back are not seen in IBS and hence least likely.
- Looks less likely to be a kidney disease as there are no urinary complaints that you have mentioned.
- Whether this is serious/cancer or not can be commented upon only on clinical evaluation and investigations which I will discuss below.
- No mention of any bloating, vomiting, constipation or loose stools or changed bowel habit or loss of appetite and/or weight.
> Since the pain is in the left lower pelvic area, sometimes going to the left testicle, left hip area and left lower back I would consider the possibility of a few things as follows and get investigated accordingly:
- MRI of the spine for detecting any prolapsed inter-vertebral disc, nerve root compression or any other pathology.
- X-ray of the abdomen in the standing position.
Ultrasonography followed by CT scan if needed.
Colonoscopy and
Upper GI Endoscopy since you have acidity related problems, a proper correct diagnosis and colonoscopy as you have left lower abdominal pain.
Clinical evaluation by a General Surgeon would help in addition to the investigations and you get a proper diagnosis and treatment plan.
Anxiety is the root cause of many problems and hence should be kept at bay at any cost if you want trouble free life.
I hope this answers your query, please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Get investigated
Detailed Answer:
Thanks for your detailed info again.
As detailed above, I tried to correlate all the symptoms that you have noted well and have said about their simultaneous appearances sometimes.
The pelvic pain as you noted just now also increases on sitting down indicating
my suspicion of prolapsed disc or so causing this.
IBS: as the name suggests is irritable bowel syndrome where there has to be a factor that irritates the bowel to cause the pain and other symptoms and all the investigations have to be necessarily normal. This is very important part of IBS.
The tests of stool, blood, urine, ultrasonography/CT scan of abdomen. colonoscopy and if required further tests like barium studies should be normal to label one to be suffering from IBS.
The symptoms of the other diseases you have mentioned like bowel cancer, cancer of bladder or kidney problems will be indicative of the problems. Intermittent pain the way you have explained may not be fitting into these diagnosis. A high degree of suspicion and related investigated alone can get you the correct diagnosis.
I hope these details help you, please feel free to discuss further if you have relevant queries.
