Can be angina.

Detailed Answer:

Have gone through your details and i apppreciate your concern.
Off and on pain in Right shoulder can be angina or due to cervical problem.
However in cervical spondylosis pain is usually constant and persists through out the day.
From your history most likely cause of pain can be angina.Angina is pain due to compromised blood supply to heart.
I would initially suggest you to go for few baseline tests of cardiology like
stress test and 2d echo to rule out the possibility of angina.
Combined together both these tests have more then 85% sensitivity to rule out coronary artery disease.

If angina is ruled out then MRI neck should be done to see if the pain is cervical or muscular in origin.

Treatment would be given accordingly.
