What Causes Intermittent Substernal Chest Pain?

Question: I have chest pain for 5 months, it is substernal, mainly at night but happens during daytime occasionally , lasts for 5-8 hours , comes and goes , it happens sometimes when I stand up and walk for a short period but when I get on the treadmill and run for 30 minutes it totally goes away , I had EKG, CXR and they were normal , I had stress echo which was normal , d dimer was normal , no HTN , no DM , no family hx up to my knowledge , never smoked , LDL 155, HDL 37, on Statin for the last 3 months , concerned about cardiac cause

Your complaints are not caused by cardiac problems.
They can be caused by costochondritis, spine or joint problem, or it can be simply muscle pain.
I would advise you not to worry and visit a specialist and check for above mentioned problems.

But why the chest pain gets worse with exertion mainly ?

Not cardiac cause

Detailed Answer:
Dear Sir,

Cardiac cause was ruled out with cardiovascular tests.
Sometimes joint and spine problems also have some relationship with physical exertion.
As you have pain during night, I would advise you to see gastroenterologist as well, because oesophagitis also can cause similar pain.
