What causes involuntary tremors in legs despite being on Ciplar?

Pls suggest treatment and any test require

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If your mother would have presented to me as an outpatient,I would have mentioned about these causes of leg tremors-
1)Anxiety
2)Essential tremors
3)Drugs induced
4)Thyroid disorder
5)Neuropathy
6)Excessive caffeine intake or abrupt caffeine withdrawal.
7)Parkinson's disease
8)Orthostatic tremors

So,to rule out the above causes,please let me know the following details-
1)Does she remain frequently anxious or stressed?
2)Is she having a Thyroid disorder?Is she having any other disease like Diabetes,hypertension or anything else?
3)Are her tremors more prominent in sitting,standing or walking position?In what position, her tremors improve?
4)Does she have tightness in limbs and slowness of daily routine activities?
5)Is she on any medicines?If yes,please list them.
6)Are tremors symmetrical in both legs or more in one of the legs as compared to other?
7)Is there a history of excessive caffeine containing products(tea,coffee,cold drinks,chocolate,chocolate ice-creams) or abrupt caffeine withdrawal?

Thanks for being in follow up with details.

I assume that the medicines you have listed must be on her schedule for many years now.Since when she is having the leg tremors?
If tremors are of recent onset,these medicines are not likely to cause the tremors.

I suggest you to make her undergo a Thyroid function tests and Fasting blood sugar.



Coming to treatment part,please let me know about the composition of croza and if it is not clonazepam then, tab. clonazepam 0.5 mg twice a day should be added to the existing dose of ciplar and response should be observed.It should bring improvement. If croza is clonazepam,please let me in what dose she is taking it?

Tremors is from last 5-6 months
She is not suffering from sugar and thyroid
Caroza is Ranolazine Extended release tablet

Clonazepam should be added.

Tab. clonazepam 0.5 mg twice a day,should be added to the existing treatment,and the response should be observed.I hope to see her tremors improve with clonazepam. Ciplar should be continued in existing doses.
