What Causes Irregular Menstruation After An Abortion?

Question: I had abortions 2 weeks ago and I was pregnant for about 6 weeks now my menstruation cycle is disturbed it's come again after 20 days so I will again have ultrasound will it detects after 2 months that I had abortions. That abortion was secret so will it be seen in any tests? Pls reply

Menstrual cycle usually appears anytime after 6 weeks of termination of pregnancy. It is likely, though not very common to have irregularities in first few cycles.

Ultrasonography is advisable in such cases to confirm that the abortion was complete. If any products of conception are noted within the uterine cavity, it indicates incomplete abortion and requires evacuation of the uterine cavity. However, it is very unlikely in your case since you had bleeding free period for 6 weeks.

Once the uterine cavity is empty, it is rather difficult for any lay person to detect the evidence of termination of pregnancy performed 2 months ago. However, any Obstetrician or ultrasonologist can notice the size of the uterus bulker than non-pregnant state and infer that you were pregnant some times in past.

Please upload your ultrasonography reports and images so that I can opine and advise you further.

Your reply was quite satisfactory. I don't have reports but I do remember that the foetus size was 1.7cm (7 weeks).I had medical abortion and 20 days of that I went for sonography. The doctor there confirmed that the uterus was clear and no remains were left. I menstruated 4 weeks after the abortion and after that in every 20 days. Will 1.7cm long foetus affect size of my uterus? Will it be evident in my next ultrasound?

It is a good news that completeness of your abortion was confirmed by ultrasonography. I would appreciate if you upload the reports and images of this ultrasonography. You may have to keep watch on any secretions from the breast.

Since you have not mentioned about foul smelling discharge, I have not considered the possibility of infection.

The menses that appeared 4 weeks after abortion seem to be absolutely normal.

How was the bleeding which started 20 days after your menstruation? If it was very little bleeding, it is likely that this is Mittelschmerz. Mittelschmerz is some amount of bleeding, may be with some pain in one side in the flanks taking place at the time of the egg release. Fourteen days after that you get normal menstruation.

If the bleeding is minimal and has stopped, you may just observe your next menses. Then you can revert back to me.

If you do not want immediate pregnancy after marriage, it is better that you start oral birth control pills which in addition to avoiding pregnancy will help you regularize the menses. Start these pills from the second day of the menses.

I am still worried about the size of my uterus. Will it be evident in my next ultrasound that it has enlarged due to 1.7 cm foetus that I carried

You need not be. Unless specifically looked for, no one would suspect pregnancy. It is now almost three months.

The uterus is enlarged no doubt, however it is very minor enlargement and it comes to normalcy by 3 months.
If completeness of your abortion is confirmed by ultrasonography, you need not go for ultrasonography now.

If the bleeding is still continuing, you may have to get investigated.

Now, XXXX, please ask your next follow up question after 2 days as I have lost my sister and I am travelling out of station where I will have no access to internet.
