What Causes Irregular Menstruation?

Question: as far back as i can remember i have had menstrual issues-irregular periods (2xmonth or different times/month), extremely heavy and extremely painful. last year i was having a ridiculous amount of shooting pains in my lower pelvic region and it got so bad that i finally went to the ER and i was rushed into surgery to remove a large dermoid cyst, my entire right ovary, right tube and all. after i started healing, i started feeling similar pains and an ultrasound showed a very small cyst that eventually went away. i feel like my gyno isnt taking me serious when i say i feel like something is still wrong. i have almost constant dull to sharp lower pelvic pain from hips up to lower stomach on both sides, bloating/gas-sometimes feels like a sloshing feeling and wearing pants can be uncomfortable at times bc im pooched out, i had to have a colonoscopy 6 months ago because i had a little blood in my stool and my bowls fluctuate btwn constipation and diarrhea, ive started getting periodic bad headaches, i'v put on about 45lbs the last 18 months, even if i get a full nights rest and wake up rested-by the end of the day i am to exhausted to do anything other than curl up in bed or on the couch and sleep, and ive woken up a couple of times lately with my breast swollen and extremely sore-especially on the outside of the left one, but there isnt any lump or anything and about 3 months ago they tested my thyroid and the levels were lower then normal so they had me temporarily take some medicine to get them back up. Im just not sure what to do with all this because i can feel that something is up.

Overall, I feel you are suffering from hormonal imbalances as evidenced by the irregular menstruation, abnormal thyroid profile, ovarian cyst formation, recurrent breast tenderness and the general weakness.

The pelvic pain could be due either to an enlarging cyst, pelvic inflammation, infection, pulling effect due to scar formation inside etc. You may get follow up trans-vaginal sonogram to know if any abnormal finding is present.

The bowel habit of alternate constipation and diarrhea without any positive colonoscopy findings is suggestive of "Irritable Bowel Syndrome.' Hormones can be a causative factor for this condition also.

Weight gain which is abnormal is also suggestive of endocrine cause. This excessive sudden weight gain in itself causes weakness. When abnormal thyroid profile is diagnosed, medication is better taken for lifetime to maintain the hormone levels within normal limits.

Please see an endocrinologist who can further refer you to specialists like physician, gastroenterologist and so on as needed. I strongly believe correction of the abnormal hormone patter after identifying the cause, maintenance of proper weight, regular diet and exercise will help solve the issues. To control the bowel habit, you may also need relaxation exercises, counselling and anxiolytics.
