What Causes Irregular Periods Along With Abdominal Pain Just Before Periods?

Question: Hello Doctor,
This is regarding my mothers medical issue.
Age - 49 years
Height - 5.1
Weight - 101
She has got these following problems.
1. Periods reoccur every 15 days if birth control pills not taken. (Sometimes)
2. Experiencing severe pain before start of periods and continue for three days.
3. Heavy bleeding for 6 days.
4. Sometimes severe pain in the back after bleeding stops.
5. Sometimes severe pain but no bleeding.
6. Repeated Giddines.

Adenomyosis with anaemia.

Thanks for writing to us about it.

I can understand your concern, though.

I have worked through your mother reports and she is having adenomyosis with fibroid and anemia.

Obesity is also a problem in her example.

All the symptoms which you have noted is due to adenomyosis and giddiness may be due to anemia.

In my view, she should consult a gynecologist and continue her birth control or depo-provera but hysterectomy is the best treatment for her.

Come after my advice before going for hysterectomy-

-She should cut her weight by the strict diet control and walking.
-She should start taking iron, calcium and continue her birth control tablets.
-She can take mefenaemic acid for pain
-Do some yoga and meditation

She had met a doctor, and got information that this said problem would be cured if menopause occurs. Is it true?
when could we expect the menopause stage.
Is Hysterectomy safe at this age and weight , are there any side effects.
Are there any other solutions through medication.

Menopause occur between 40-50 yrs.

Hi,

Yes, it is true that adenomyosis reduce after menopause because they are hormone dependent tumor and after menopause estrogen level decreases (estrogen dependent tumor).

Menopause normally occurs between late 40 and early 50yrs. (Means she is about her menopause) but it may vary person to person.

Hysterectomy is having side effect similar to any other surgical procedure, but yes, as your mother weight is high, so some anesthetic complication is more with her, but do not worry about it.

Non surgical treatment of her problem through hormonal pill like progestin only pills, depo- provera, progesterone containing IUCD. Uterine artery ablation is another procedure use for its treatment.

Definitive treatment is hysterectomy in her example.

It is not a serious matter, but it's become a serious issue when she bleeds profusely (during her menses) due to adenomyosis.

Hence the decision of hysterectomy depends upon whether her bleeding is responding to hormone pills or not.

As she is already anemic, control of bleeding is very significant.

so she can wait for menopause if her bleeding is under control, otherwise she should go for immediate hysterectomy.

The following are medications she is taking right now
Once a day
1.Metosartan 25
2.Novelon
3.Dexorange
4.Conviron
5.Eltroxin 100
6.Primosa100

Sometimes
7.allegra 120
8.Trappic pills upto six a day while heavy bleeding

As she says:
There is no much difference in bleeding, and its always heavy and painful.
so... please look at the above medications if any contraindications or side effects.
Please do suggest if any other beneficial medications.

Can you suggest in detail about non surgical method suitable for her.

Mirena ( levonorgesterol) IUCD can be used.

As she is suffering from hypertension, she should consult her treating physician about use of novelon.

She can use progesterone containing pills, Depo-provera injection or minera IUCD instead of novelon.

If she is bleeding so much then hysterectomy is the best treatment for her.

But if she is not willing for hysterectomy, then she can use minera IUCD.
