What Causes Itching All Over Body At Night And Redness Of Skin?

Question: I have itching problem for past 4 to 6 months speacially at nite...Lot of Heat and redness comes on skin...and sometime swelling also.

Even itching happens on scalp.

I got married 1yr 5mnths ago and my husband has same problem.

I am Dr. Kakkar. I have gone through your concern.

I would keep a possibility of either Scabies OR Urticaria/Hives.

Scabies seems the more likely diagnosis. It usually presents as nocturnal itching. There can be lesions of scabies(resemble mosquito bites), at certain sites like chest, abdomen, genitals, buttocks etc. There is usually a family history in close contacts e,g husband and wife.

I would like to gather some more information from you in order to be able to guide you better.

-Do you people notice that the itching is generalized OR is it specifically more severe at certain sites like thighs, genitalia, buttocks, chest, abdomen etc.

-Are there any skin lesions to be seen like itchy nodules OR lesions resembling mosquito bites over the skin OR are they just like Hives which disappear completely within a few hours, without leaving a trace?

-Can you upload a picture of the skin lesions?

Treatment of scabies is by single application of a 5% permethrin cream from neck downward and keeping it overnight followed by bathing next day. All the affected members should be treated in the same way. A single application is curative.

An OTC antihistamine like cetrizine 10mg once daily in the evening OR Allegra 120mg (which you have been already taking) provides symptomatic itch relief.

also would like to tell u that if i remove forcefully my rings or bangles then my fingers or hand swells for 1-2days.

Urticaria with Angioedema

Detailed Answer:
Thanks for the additional information

I would keep a possibility of Urticaria with Angioedema (swelling of hand and fingers). While Urticaria presents as generalized itching and superficial skin welts/swellings, Angioedema or Quincke's edema on the other hand, is deeper and presents as rapid swelling of the dermis, subcutaneous tissue, mucosa and submucosal tissues. There might be a burning sensation in Angioedema rather than itching.

The mechanisms involved in the development of urticaria and angioedema are the same, the difference being in the depth of skin involvement.

Angioedema can result in swellings that can last for days whereas urticaria rarely lasts longer than 4-6 hours. Angioedema can be associated with urticaria or without urticaria.

Urticaria OR hives can occur due to various causes like(to name a few):
--Infections(bacterial , viral, fungal)
--Drugs(Pain killers/ antibiotics etc)
--Certain foods and food additives/preservatives
--Physical Urticaria: Cold temperature, Heat, Pressure(Prolonged Standing, Sitting), Dermaographism etc.
--Cholinergic Urticaria: in response to strong emotions, exercise, spicy food etc --Autoimmune disorders like SLE, RA etc.
--A few cases have no underlying cause and are known as idiopathic urticaria.

I would suggest you to undergo a few lab tests like Complete Hemogram, Urine Routine, Stool for ova/cyst, C4 levels.

An oral antihistamine like Fexofenadine (Allegra 180 mg tab) OR Loratadine (Alaspan 10mg tablet) once or twice daily for a week would be suitable. They are non-sedating and wont affect your alertness level.

A topical soothing lotion like calamine would help you symptomatically.
