Brief Answer:
May be 'genital herpes'; need more information on that !!!

Detailed Answer:
Welcome back and thanks for the consultation at my virtual clinic!

It seems to be Genital Herpes infection of mild intensity. It is a disease that spreads through sexual contact and is very common disease. It has a typical waxing and waning pattern. We need to treat the active lesions on the genitals as they are the main source of viral shedding off. Antivirals work good for that. The viral active shedding and spreading phase is succeeded by the dormant phase. The virus loves the nerves of the patient to live for an indefinite periods of time.

We can't eradicate Herpes but we can save others from getting the infection through the use of antivirals (the prescription drugs that lessen the intensity of the infection during an outbreak of active viral shedding phase)

The dry skin on the penis on zoom-in looks like to be due to scales formed on the cold sores and healing ulcers.

Avoid sexual contact until your penis skin is cleared of the symptoms. You are at greater chance of getting other sexually transmitted diseases like HIV.

I need some clarifications so that I may be able to comment on the issue with more clarity. So, Please answer these questions:-

Do you feel painful if urine touches anywhere on your penis??

Do you had this kind of symptoms in recent past??

Do you feel on touch, any swollen lymph nodes in groin areas??

Do you have pain and/or itching on the penis?? If yes, where?? On the shaft of penis on the dry spots or on the glans penis??

Do you have any other symptoms anywhere on the body like any ulcers or sores inside mouth??

For how many days do you have this issue??

Pain anywhere inside scrotum and testes or groin??

Can you remember when you had a sexual intercourse without condom last time???

Have you got tested for any laboratory investigation regarding this disease??

Do you have diabetes??

Note:-
For itching on the scrotum you should apply Clotrimazole + Hydrocortisone ointment. It is the jock itch and is caused by the Tinea fungus. Keep the hair from the genitals shaved in a timely fashion.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion.
