What Causes Itchy Rash On Shoulders?

Hello
Rash only on shoulders or on other accessible parts of body is usually seen in prurigo nodularis.
It is a condition where there is immense desire to itch which leads to rash formation.
I would like to know few more details.
1.Rash appears first or first there is itch leading to development of rash?
2.Do you have history of any other allergic disease like asthma in past ?
2.Any thyroid disorder ?
Please answer the above and if possible upload a photograph of the affected area.
