What Causes Itchy Red Spots In The Groin Area?

Question: 54 yr old male. About one year ago I noticed a few small red itchy places around my genitals and inner thigh. The itch would come first and then I would notice the red spot. They would sometimes develop into a small 1/16" white spot that would ooze a liquid if scratched.
These places would go away after about a week but new ones would come back and began to spread to my lower legs, feet, forearms, trunk area. The spots are random and have no pattern.
I hadn't changed anything in my foods or daily activities but these just began to appear.
Dermatologist biopsied them and said that it wasn't a allergic reaction. They think it's urticarial dermatitis and have given me high doses of antihistamines, prednazone and steroid shots on two occasions. The steroid shots are the only thing that has consistently worked.
It's been a year and now I'm on mycophenolate for the last four months. They seem to have gotten better but they're still coming back and just won't go away.
Again, the primary locations are forearm, innerthigh pelvic area and lower legs. I have had 2 or 3 randomly appear on my penis. None on hands, face or neck. Can you please help as I'm quite discouraged.

Thank you for your reply. I have tried numerous times to upload pics to the "update reports" section and they do not upload.
Additional information about my condition is that they originally tried to treat it for scabies because it looked similar to them but that was not successful. The places I have are significantly fewer than what I had early on this process. Generally they Are in areas that are warm(crotch, Beltline, top of sock area) and contact with clothing. They do not bother me during my sleep and generally are better in the mornings than in
The afternoons.
Can you advise how to upload the pics that you requested?

Attached are two pictures of the red bumps. One is on the inner thigh and the other is on base of penis. This is the first one I've had on the penis shaft. These are generally about 1/16" in diameter. As mentioned earlier, back in the Jan-march time frame of this year, I would have MANY(50 to over 100) of these along my lower trunk area, lower legs, forearms, underarms, top of feet, and crotch area. I would first notice a itch in the area and then would find a red spot. Not all red spots had the little raised area that appeared like a small blister though. Most would be little red dots/splotches(all about 1/16") and most all would have a minor itch to start with. They last about a week or maybe a little more and then they're gone.
Right now, I have about 5 on my body, two of which are in the attached photos. Having only 5 is great but that fact that this is still coming back a year later is concerning.

Possibility of Scabies

Going by the distribution of skin lesions and clinical symptoms I would keep a possibility of Scabies.
I would suggest you to take a single 12 mg dose of Ivermectol tablet. To be repeated after a week.
In addition I would suggest you take an oral antihistamine e.g cetrizine 10 mg once daily in evening for symptopmatic relief from itching.

In my original question, i failed to mention that they did originally treat me for scabies including the ivermechtol tablet and as stated, heavy doses of antihistamines ie. daily i would take zantec, zertec and one other that i've now forgotten the name. My spouse who has been with me the entire time shows no indications at all.

the locations are always 1/16" or smaller, generally begin with a minor itching sensation and then if its in a area that i wouldn't generally rub off(as with clothing) it may produce a small postule. In my reading about scabies, it seemed to talk about tracks as if from burroughing and there have never been anything like this. each spot is individual and stays about a week, sometimes longer depending on how much it gets irritated by clothing or scratching, which i try to avoid.

It's very concerning that these areas still come up after one year.

I would like to consider another possibility of either an Id reaction or prurigo simplex in my differential diagnosis.

How has the response to topical steroid application and Oral antihistamine? Have you tried that?
