Brief Answer:
As discussed in details below.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/45 - whitish rash/inflammation on the side of scrotum - frequent itching - tried baby powder - not getting better - wants suggestion ...

This is suggestive of Jock itch.
The occurrence is due to moister that retains due to typical position of this area, remains closed all the time added on with tight underwear, pants, atmosphere into which one works and so on.
Hence the basic treatment is prevention:
First of all watch for the atmosphere you live in, work in. Try to change to cooler and aerated if possible.
Use loose underwear and pants; very loose covering when at home.
Clean the area with pain water as many times as possible and make the area dry with hair dryer. Then apply liquid povidone iodine and let it too dry up before you start to go for the work. This allows sterilization of the area and avoidance of secondary infection.
Repeat the same after coming home from work and apply anti-fungal liquid at bed-time. Do not use any ointments as it does not allow evaporation of the bodily fluids.
Get blood sugar done to rule out diabetes.
Unless the causative factors are corrected, this is not going to heal up.

I hope this answer helps you, get started and see the results and give feedback.
You may post the clear and focused pictures if possible as it helps for on-the-spot diagnosis and proper management.
Stop using any soaps or sprays that contain chemicals.

Wishing you an early and permanent recovery.
