Question: I have recently developed a pronounced, large swelling in the upper left abdominal quadrant (area of liver) along with accompanying dull pain in the back anterior to the bulge. The bulge is soft and pain free to probing. I am type ii diabetic, with HBP, smoke 1 cigar per day, non alcoholic,

Hi.
Thanks for your query, albeit short and specific.
A soft and painless bulge in the left upper quadrant with dull pain in the back in a type II diabetic with high BP can be due to the following reasons:
- Psuedocust of pancreas.
- Hernia
- Entrapped wind in the large colon with fecolith or so.

I would advise you the following ASAP:
Ultrasonography of the abdomen and CT scan to help better definition.
Clinical examination by a General Surgeon.
Tests of blood- routine and particularly for diabetic control, amylase, lipase, kidney function and liver functions.
Tests of urine and stool.

Confirmation of the diagnosis and treatment as per the diagnosis.
