What Causes Left Side Pain During Pregnancy With Twins?

Question: I am approximately 6 weeks pregnant with a twin gestation. I have worked OB for a year and a half so I am familiar with all medical terms if you need to use any. I was seen in my doctors office on Thursday of last week and he was able to approximate my dates with a date of conception being on or around 9/2/16. He was able to get a fetal pole on Twin A but was only able to see a gestational sac on Twin B as of Thursday that just passed. I have been having some left side pain that does not come and go but just remains like a constant pain on that one side so I went to the ER just to have blood work drawn and another ultrasound (abdominal and transvaginal). The ER doctor reviewed all lab work and said that it looked fine but I do not know the progesterone or HCG levels that were found but he did not seem to find anything wrong with my urine nor my blood work. The ultrasound revealed a fetal pole on Twin A with 105 BPM but in Twin B gestational sac they were still unable to find a fetal pole but it did reveal a yolk sac unlike the ultrasound that I had in my dodctors office on Thursday this week. My concern is with the pains that I am feeling as well as the no fetal pole on Twin B. I am hearing that it is common for Twin pregnancies to develop at different rates and speeds. So my question is with there no being a yolk sac being seen this early on in Twin B is this is a sign that Twin B will most likely be viable as well? Also should I worry so much about the pain that is about on a pain level of about 4 on the left side or could that just be my uterus stretching out? I have a copy of the ultrasound report and will attach it if you need to review it. I do see in the report where the radiologist stated that I have a Corpus Leutem cyst on both ovaries which I know need to be there to support my little babies as they grow. I am just afraid that I may have one bad pregnancy with Twin B and that I will loose both babies. Please help!!

I understand your anxiety about the issue.
I have seen the attached reports.
There are less chances of development in twin b as it seems like a blighted ovum which is common in twin pregnancy.
This is called as vanishing twin and is the cause of pain most likely.
Take as much rest as possible and avoid inrercourse and exertion of any kind.
Also get a repeat scan after 1 week which will give maximum information.

Thank you. So with Twin B will I expel this and bleed and lose my other baby or will Twin A most likely stay put? And as as far as the yolk sac I though this was an indication that the pregnancy possibly could be viable bc I thought with a blighted ovrum nothing would be in the sac at all?

Yes you are right

Yes you are right that blighted ovum has nothing in it so it is a pregnancy only bit presence of heart rate is a must to label it viable.
The aborting twin usually causes no problem but at times can lead to demise of other twin too.
So you need to have rest and precautions along with medicines as told by your doctor.
Next week scan will sort out everything.

Thank you for your answer and help. My doctor called me today and stated that he wishes he could tell me that all will be just fine. He did however say that the pain is most likely due to Twin B and that my body my expel that sac and that most times the other twin lives on but it is just to early to tell. He says that the lower back pain and pulling feeling in my abdomen is most likely the uterus contracting so that the bad sac will be relaxed from inside there. He said I am only measuring 5weeks and 6days and we got a fetal pole on Twin A very early which is a good sign that this one will most likely stay put since they are in two separate sacs. I am praying and hoping that nothing else goes wrong and that my other baby survives this as I will be e tensely sad to lose both. I have no children and am finally pregnant and want this baby so I am hoping that the odds are not against me.
