Brief Answer:
may indicate involvement of kidneys or presence of a stone

HI, thanks for using healthcare magic

If you have been on antibiotics for less than 3 days, then it is possible that your body has not been able to mount a response as yet though in most persons there is a slight response.

Urinary tract infections can involve any aspect of the urinary tract - the bladder, the ureter, urethra, kidneys.

In most cases, mainly the bladder is involved. When there is kidney involvement (called pyelonephritis), the symptoms can be more severe.
It is also possible that there may be a stone, this causes severe pain as well.

You may need to revisit your doctor if the pain persists. An ultrasound may be needed to take a look at the urinary tract.

I hope this helps, feel free to ask any other questions
