Brief Answer:
Get an ultrasound and a culture.

Detailed Answer:
Good Day and thank you for being with Healthcare Magic!

I am sorry for your predicament and feeling of hopelessness and I will try to help you in the best of my ability.

For recurrent and stubborn infections I would recommend doing a urine, semen and prostatic massage fluid cultures. This would help us zero in on the bacteria and will tell us what antibiotic is needed to treat it properly.

An ultrasound of the the kidneys bladder, prostate and testicle with Doppler is also needed to check for any abnormalities that would make you prone to infection.

Ciprofloxacin or ampicillin sulbactam are both good drugs to treat orchitis for organisms that are susceptible to the drugs.

Once you have the result you may contact me again so I can help you further.
