Question: Dear dr, for the past 3 weeks I've been feeling like I am going mad as if my head was going to explode. I cannot focus for long periods on the computer either as the bright light affects my eyes. When I speak I can hear the echo of my own voice. My work has been very stressful over the past few weeks that I've taken some time off work, which has not really worked. I'm afraid that I am seriously going mad and am worried. I went to my locus doctor but he said that I needed a holiday and never took me serious. I felt leaving his practice feeling very disheartened and upset. I do not suffer with depression or any mental issues.please advise.

Hi Mam,

I understand your concern.

You seem to have possible Shift work Disorder.

This can cause wide range of symptoms which you mentioned.

For this I advise,you take a prescribed sleep/wake scheduling.
See that you have an average sleep duration of 7 hrs over the next two weeks.

This will improve chronobiological time of brain activity of sleep.

Take time off for at least 2 weeks from here on since your symptoms are severe enough to warrant drug therapy.

Additionally I advise you to take Tab Clonazepam 0.5 mg once daily at bedtime for 3 weeks and Cap Melatonin Once daily at bedtime for 3 months.

This should improve both your mood and sleep pattern and reduce the symptoms.

Dear dr XXXXXXX

Thank you for your reply but I don't work shifts, I work 9-5pm, hence is this why you called my symptoms shift work disorder. Does this imply some mental health issue?
I would also add that my sleep pattern is good with an average of 8-10 hrs per night

SWD is not a mental illness.

Hi Mam,

I understand your concern.

SWD is not a mental illness.

Its an over exhaustion of brain functional activity.

SWD need not necessarily imply working over nights.

It usually indicates inadequate rest to the mental activity secondary to excess stress related to working environment,both physical and psychological.

Sleep pattern can be initially normal in few cases.

So I advise you to continue the prescribed medications.

I have been admitted to casualty as the consultant has serious concerns regarding my headachrs. CT scan or MRI is to follow.

I advise you go for MRI if any,over CT scan.

MRI has excellent superiority over CT scan.

Your doctor is viewing your headache from a different perspective.

But at the end of the day,its always safe to have an investigation to rule out Organic brain lesion.
