The gradually decreasing use of the right hand and arm is likely to be due to a nerve root compression at the level of cervical spine due to arthritis in the neck.
If there is a considerable decrease in the muscle strength then you need to consult a neurologist as a nerve root decompression may be needed.
You can also take muscle relaxants and consider going for a cervical traction in addition to gabapentin that you are already taking.
