Question : i was just diagnosed with sinus tachychardia. I already take 100 milligrams of Lostartan QD, my Dr. wants me to take Atenolol 25 mg at HS. My blood pressure presently is 100/65 with a HR of 102. Should I take the Atenolol?

Hello,

if you were not taking losartan already then your blood pressure would be just fine. For hypertensive patients though, 100/65 is a low blood pressure, if this is your regular blood pressure. The target levels of systolic blood pressure should not be lower than 125mmHg (with some rare exceptions, like aortic aneurysms, etc). Atenolol would be OK if all causes of tachycardia had been excluded like fever, anemia, blood loss, thyroid disease, etc

If I had to give atenolol to a patient with low levels of blood pressure, I would have advised the patient to stop the other drug. I couldn't possibly know the reasoning behind your doctor's decision, so you'd better discuss the matter with him/her. If you have any symptoms of hypotension like faintness, dizziness, etc then you should definitely stop one of the drugs.

Follow up: Dr. Panagiotis Zografakis (15 hours later)
My BP in the mornings is higher ( anywhere from 145/95 to like this AM 135/85 with a HR of 85 ) after I take the Lostartan it goes down. Sometimes I think too low. Feel "funny" at times. My HR will go down to 70's - 90's. I am a diabetic with anxiety as well if this makes a difference. Just started Paxil for anxiety. My Dr. Wanted me to take the atenolol at HS to lower the BP in the mornings which is when it's the highest.

a 24 hour Holter measurement or serial measurements would help

I got the impression that you had only low readings. If you do have high readings then atenolol would help, although it may worsen your glucose control a bit. A 24 hour Holter blood pressure assessment would help to find out if you do have very high or very low blood pressure during the day (and night). Alternatively serial measurements for a few days will also help.
