Brief Answer:
You need proper clinical exam & investigations.

Detailed Answer:
Hello XXXXXXX
Thanks for writing to HCM

Your RBC count is low.In adult males,it is about about 5 to 6 million per microliter.
Since both liver and spleen is enlarged, you need proper clinical examination and further investigations.
Investigations include routine hemogram,RBS(Random blood sugar),LFT(Liver function test),RFT(Renal function test),Lipid profile,Ultrasound of abdomen.Further investigations can be done if needed.

Liver and spleen may be increased due to infection. It may be acute or chronic.
Infection is also supported by increased ESR.It may be due to typhoid, malaria etc,but you don't have complain of fever.
Liver may enlarge due to past history of malaria.

It is important to know other parameters like WBC count.Peripheral smear of blood should also be examined.

You may need iron supplements for low RBC count.

You should contact your physician.Proper treatment depend upon clinical findings and investigation reports.
You may need to take antibiotics.

Get well soon.
Hope I have answered your question.
Further queries are most welcome.

Take Care
