Question : my wife tsh last year was july was 19 and now it is 3.39 without any medicine is it possible

Yes, it is possible for TSH to drop like that. TSH is a dynamic hormone and it changes according to many factors. Sometimes when someone is recovering from an illness, TSH can go high. It slowly come down to normal. So it is possible this way.
What is her Free T4 level. It would be good to do an Anti TPO as well
