Question: I doctor . I have been having low energy , mental confusion, day time sleepiness and low tolernace to exercise. I got my CBC and B12 tested . Reports are attached. B12 was on the lower side . Could it be because of some digestive disorders like celiac or crohn's . If yes what kind of screening should I do for those disorders. Also what should I do for B-12 deficiency

Brief Answer: You may have underline depression Detailed Answer: Hi Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query. I have seen your reports of CBC and B12. Both reports are normal and there is no problem in your gastric system. I think, you have underline depression or anxiety disorder. Sleep disturbance, letharginess, decrease concentration, fatigue are symptoms of depression or anxiety disorder. In that case, you need proper dosage of antidepressant. Answer few questions, so i could make a diagnosis in your case: 1. How is your mood these days? 2. Do you have any stress in work or family life? If you are on any antidepressant, tell me, so i could give you further advise. Regards Dr. Abhishek Kapoor Psychiatrist

Hi Doctor. I have actually being diagonsed with anxiety disorder and panic attacks. I am have been on and off of various anti-depressents . Fludac and lonzep are two names that come to my mind. Since the symptons are now interferring with my personal and professional life therefore I need to sort it out and I am having all possible tests to determine the underlying cause of these symptons. Also if symptons are anxiety related as you are suggesting than all my anxiety is basically healthy anxiety and I am sure if physical causes are eliminated if any then I am sure it will reduce my health anxiety too. I am consulting Dr XXXXXXX XXXXXXX from Hirandani hospital for this matter but I have not yet started on the meds as I am vary of the side effects. So basically all my stree is health related only.

Brief Answer: Your physical symptoms are due to stress Detailed Answer: HI Your physical symptoms are due to stress and stress can never be healthy. Let me explain it. Stress can affect every part of the digestive system. Digestion is controlled by the enteric nervous system, a system composed of hundreds of millions of nerves that communicate with the central nervous system. When stress activates the "flight or fight" response in your central nervous system, digestion can shut down because your central nervous system shuts down blood flow, affects the contractions of your digestive muscles and decreases secretions needed for digestion. Stress can cause inflammation of the gastrointestinal system, and make you more susceptible to infection. Stress can cause your esophagus to go into spasms. It can increase the acid in your stomach causing indigestion. Under stress, the meal in your stomach can shut down and make you feel nauseous. Stress can cause your colon to react in a way that gives you diarrhea or constipation. Although stress may not cause stomach ulcers, celiac disease, or inflammatory bowel disease, it can make these and other diseases of digestion worse. Same problem is in your case. You are thinking that your anxiety and physical symptoms are different, but there are interconnected with each other, basically in your case, physical symptoms are due to stress. You may have stress due to your professional or personal life and this stress is now effecting your body. Let take a example: Psychological anxiety can cause increase heart rate, tremor, sweating, headache etc, but in that case, symptoms are not due to any physical disorder, but due to psychological issues. Body is control by our mind. If our mind is disturb,so body is. I think, you should take medicine prescribed by Dr XXXXXXX and do relaxation exercise that would keep you in good shape. Gradually these physical symptoms would improve. In case, you have more query, you can ask. Regards Dr. Abhishek Kapoor Psychiatrist
