Brief Answer:
Low testosterone

Detailed Answer:
When a man is diagnosed with a low testosterone problem, tests are run to first determine the cause, and only then is treatment begun. Treatment is typically directed at the cause.
Sometimes, a careful decision is made by the hormone specialist doctor ie endocrinologist to start Testosterone replacement therapy. One of the ways of doing that is to give Testosterone injections.
It is possible your son was given this as a shot. However I cannot say this with certainty. You will have to see the actual prescription to confirm.

When I see someone like your son in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your 3 month glucose average)
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH
Free T4
FSH
LH
Prolactin
Serum Cortisol at 8 am
Total Testosterone
SHBG
25 hydroxy Vitamin D
Karyotype
Ferritin

None of these tests require any fasting and can be done at any time of the day.
Sometimes an MRI of the pituitary is required based on the results of the above tests
