What Causes Lower Abdominal Pain Along With Frequent Urination?

Question: HELLO DOCTOR, IAM HAVING SLIGHT PAIN IN LOWER ABDOMEN. THERE IS PAIN IN BOTH THE KNEES AND THIGHS AND LOWER BACK. I FREQUENTLY PASS URINE. IAM 46 YRS OLD. 1 YR BACK WHEN I TOOK ULTRASOUND ABDOMEN IT REVEALED PROSTAMAGALLY. IAM TAKING URIMAX D SINCE THEN.IS THIS RELATED TO PROSTRATE OR URINARY BLADDER OR KIDNEY OR IS IT SIMPLY URINARY INFECTION.WHAT SHOULD I DO SHOULD I GO FOR ULTRASOUND ABDOMEN OR TAKE URINE CULTURE TEST. IAM NOT A DIABETIC. MY KIDNEY FUNCTION BLOOD TESTS ARE NORMAL BEFORE 2 MONTHS.

Thanks for your query,based on the facts that you have posted it appears that you have infection of Prostate (Prostatitis)

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Is your blood test for serum PSA done recently?
2) Have you done Trans Rectal Ultrasound Scanning (TRUS)?
3) What was the size of the prostate reported on ultrasound scanning ?what was the post void urine residue reported on Ultrasound scanning done one year back.?
4) Have you consulted qualified Urologist for clinical and digital rectal examination? if yes what were the findings?

Please get following basic tests done to confirm the diagnosis.
1) Urine routine and urine culture
2) Serum PSA This is screening test to rule out cancer of the prostate which we advice at this age.
3) Culture and sensitivity of urethral discharge obtained after digital rectal examination and prostate massage .This will help to rule out Prostatitis.
4) X-ray of Lumbosacral Joint and spine

In the mean while start taking broad spectrum antibiotics like Cefexine along with urinary antiseptic like Nitrofurantoin twice daily along with Urimax that you are taking ..Later on switch on to appropriate antibiotics as per culture report.

Further treatment and any more tests to be done will depend upon results of these tests

ACCORDING TO MY ABDOMEN SCAN REPORT TAKEN ON OCTOBER 2013, PROSTRATE SIZW- 3.7 * 4.1*3.4 CM (VOL- 27.5 CC). URINARY BLADDER- MILD DIFFUSE IRREGULAR BLADDER WALL THICKENING WITH FEW FLOATING INTERNAL ECHOES. RESIDUAL URINE- 20 ML.
PSA TEST TAKEN ON OCT 2013- PSA- 0.765 NG/ML, FREE PSA- 0.344 NG/ML ,
F.PSA/T.PSA RATIO- 44.96 %.

WHAT SHOULD I DO. MY BLOOD UREA AND CREATININE IS NORMAL. I HAVE CONSULTED A GENERAL PHYSICIAN. HE HAS ADVISED URINE ROUTINE AND CULTURE. URINE ROUTINE TEST IS NORMAL. PUS CELLS- 0 TO 2. AND NO BACTERIA IS SEEN. NO URINE SUGAR. PROTEINS ABSENT.CULTURE TEST RESULT NOT YET COME. PAIN IS CONTINUING IN KNEES AND LOWER ABDOMEN AND LOW BACK AND THIGHS.IS ANYTHING SERIOUS OR ABNORMAL.I HAVE ALRERADY TAKEN NORFLOX AS PER ADVICE OF MY PHYSICIAN.

Thanks for feedback Your reports of investigations done one year back are within normal range .I still do not understand the rationale behind prescribing Urimax .

I would suggest you to consult Urologist if you have any issue as regards urinary symptoms .

You need to consult Orthopaedic surgeon for knee ,back ache etc to rule out any orthopaedic cause for symptoms .

I TOOK ULTRASOUND ABDOMEN AND XRAY OF SPINE.PROSTRATE SIZE- 3.2*3.8*3.3 CM (VOLUME- 21.00 CC).POST VOID RESIDUE- 15 ML.THE REST OF ORGANS LIKE URINARY BLADDER, KIDNEY, SPLEEN ,PANCREAS, GALL BLADDER BEING NORMAL. LIVER ONLY SHOWING FATTY LIVER.
X RAY SHOWED LUMBAR SPONDYLOSIS. NOW MY PHYSICIAN HAS SUGGESTED TO TAKE MRI SPINE. MY PAIN HAS NOT REDUCED. NOW MY DOCTOR SUSPECTS DISC PROBLEM. I HAVE ANOTHER PROBLEM APART FROM FREQUENT URINATION. I PASS STOOLS NEARLY 4 TIMES A DAY.MOTION IS NOT LOOSE BUT OF NORMAL CONSISTENCY. MY DOCTOR ADVISED STOOLS TEST SOME TIME BACK BEFORE 2 MONTHS. IT SHOWED NO ABNORMALITY, WITH OCCULT BLOOD AND NORMAL BLOOD NEGATIVE. WILL THERE BE ANY PROBLEM IN COLON. WHEN I ASKED ABOUT COLONOSCOPY TO MY DOCTOR MY DOCTOR SAID THAT THERE IS NO NEED FOR IT AS STOOLS TEST IS NORMAL. AND OTHER PARAMETERS LIKE ESR AND OTHER BLOOD TESTS BEING NORMAL.HE ALSO RULED OUT HERNIA, SAYING THERE WILL BE A BULGE IN LOWER ABDOMEN REGION WHICH IS NOT IN MY CASE. BUT MY KNEE PAIN AND THIGH AND HIP PAIN PERSISTS. WHAT DO I DO.

Your problem of pain in both the knees ,thigh and legs seems to be due to some pathology in spinal canal like prolapse inter vetebral disc and needs to be evaluated by doing MRI.
