Question : Is there any thing that could cause low vitamin d and b12 high levels of serum and 24 hr urine dopamine and right lower quadrant severe abdominal pain lasting 48+ hrs and coming through 100 mg of ultram. Ruled out appendix and ovarian cyst at er.

Right lower abdominal pain along with high Dopamine level in urine and low vitamin D level can be caused by carcinoid of the appendix or neuroendocrine tumor of right ovary.
Carcinoid is a tumor which looks like a malignant (under a microscope) but acts like a benign tumor. It belongs to hormonal active tumors. This means that it may produce hormones (dopamine is one of possible hormones) which can be detected in the urine. Some of this hormones can also affect Calcium levels or Vitamin D levels.
The pain is directly caused by intestine invagination due to tumor presence.
It is also possible that the tumor is located in the ovary.

I suggest you to consult endocrynologist and have abdominal CT with oral contrast done.
