What Causes Lower Back Muscle Spasms?

Question: Have been having low back muscle spasm for past week....
Very debilitating,,,,was seen at ER twice.....
First time. Seen...did X-ray on low back....3views
Gave mucslular injection and prescribed oxocodone (po)
SLOWLY got better each day....but not able to function and perform ADLs....three days in and slightly twisted at waist and started all over again....
When spasmimg......unable to walk.....pelvic area is burning, painful and upper legs hurt terribly....
This time at ER,,,, was given IV Morphine and Ativan..... And sent home with follow up w PCP if not better.....script for at avian given to only be taken if spams starts.....2/day....
What is causing spasms....
(Initially last sat, what completely fine when I went to sit in an XXXXXXX style to read) when this brought about the most excruciating pain.......at which time I to ok prednisone (standing order for my anky losing spondylitis , took Flexiril, and some over the counter analgesia......
What does this sound like to you is going on?

I have gone through your query carefully, the cause of spasm at back looks to be acute flare up of ankylosing spondylitis along with bad posture. You may have drugs like methotrexate, sulfsalazine after consultation with the rheumatologist for ankylosing spondylitis.
Now, coming back to your backache my advice would be-
1. Hot water fomentation, local analgesic gel application, avoid prolong sitting and best way to take rest is lying down.
2. Drug wise I would like to treat you with Aceclofenac along with muscle relaxant like metaxolone, thiocolchicoside.
3. Phyiotherapy also helps in form of short wave diathermy/TENS therapy. So you may have it.
