What Causes Lower Back Pain Across The Kidneys And Upset Stomach?
Question: My husband is suffering from lower back pain across were the kidneys are.
He is feeling sick and has an upset stomach.
There is no blood in his urine.
He can only get comfortable when he lies down
The pain starts 30 minutes after eating.
He has no gall bladder or appendix
He does suffer from divaticulitous

My thoughts with possible explanations:
- This can be a gastric ulcer on the posterior wall of the stomach as the pain starts 30 minutes after food, back pain can be a referred pain, feeling sick and upset stomach.
Needs upper GI Endoscopy ASAP, put on PPI like Omeprazole, antacid and see the response.
- MRI of the back for possible compression on the spinal cord or nerve roots and other nearby structure pathology as the symptoms can be due to this also as he is getting relief on lying down (He can only get comfortable when he lies down).
- Clinical evaluation by a General Surgeon to start with for clinical actual examination and getting the relevant investigations.

It can be due to pancreatitis but I did not mention as per the history you have provided.
In pancreatitis the pain is very severe, excruciating, immediate reference to the back and needs hospitalization and the patient can not really take anything orally. Hence the least chances. Also you have mentioned no gall bladder. (stones slipped from Gall bladder is one of the most important reasons for pancreatitis).
The MRI of the back also shows pancreas and would have helped to make suspicion and have further studies advised,.
I am sure, by this time the investigations night have been done. If so, please post all the reports and the opinion of your Surgeon/Doctor.
Other possibilities to mention are:
Anything in the retroperitoneal area like abdominal aorta can cause such pains and are elusive to the regular diagnostic means hence investigations has a major role.
