Question: wife has lower back pain
early on vomited once and was dizzy and nauseous...could hardly stand up
thought we saw blood clots in stool once but no more
stomach or lower discomfort periodically
exhaustion and very tired
wife has bipolar disorder taking lithium, lamictal & synthroid
diahrrea and after feels like going to stool regular but never do
when getting up to go to bathroom little bowel control.....simply comes out into underpants
gets up everyday wringing wet
father had crohn's

I have gone through the medical history of your wife and understood your concerns.

Having lower back pain, blood in stool, stomach discomfort and diarrhea are indicative for intestinal problems:
- Chrohn's disease (as she is having a family history of Crohn)
- other inflammatory/infectious issues of intestines
- hemorrhoids (if the blood you saw was red)
- possible polyps or other tumoral problems of bowels.

Feeling tired and exhausted might be related to:
- her problems with bipolar disorder
- medications she is taking
- anemia
- issues with her GI tract

At this point, if I was her caring doctor would ask to run further tests to determine the right diagnose:
- complete blood work to exclude anemia
- fecal occult blood test
- colonoscopy to rule out local damages of bowel mucosa and evaluate if hemorrhoids are present
- culture of stool
- examination of stool for ova/parasites
- abdominal ultrasound

Depending on the tests results, specific treatment will be prescribed. You are also reporting that she is taking lithium which might cause the same effects on GI tract. So, you can discuss this with the doctor who prescribed lithium in order to stop it (need tampering first) or switch to other drugs.

Meanwhile, I advise to :
- give her plenty of fluids to drink
- prepare a healthy menu plan for her with healthy foods and drinks to consume on daily basis
