Question : My mother is in end stage renal failure from Polysystic Kidney Disease. She is experiencing lower back pain for the last few days. Could this be from her kidneys? Or would it just be muscle spasms?

Yes, this can be from her kidneys. Please note that policystic kidney contains a lot of large cysts. This cysts due to impeded urine flow can be the reason of recurring kidney infection.
Typical kidney infection symptoms are lower back pain (usually one side), lower abdominal pain, burning while urinating. Fever can be also present.
Of course other possibilities like strained muscle or spine disc herniation should be considered.
However as possible infection is the most serious issue, she should have it checked at first. She should have urine analysis and blood work at first. Antibiotic treatment might be necessary.
If the infection will be ruled out, she should consult orthopedist and spine MRI should be considered.
