Thanks for your query and an elucidate history.
To recapitulate: Male/29 - have a feeling of lump on the right side of the neck as you explained since you remember (all your life as you have explained) - fatty lump on right of the neck - as the base near collar bone and closure to jaw bone - can be seen as the right side is bigger than the left side - also have bilateral shoulder pain and weakness in arms - Blood, X-ray and CT scan came back normal.
I would advise you in such a situation the following:
- Examination by a General Surgeon to asses the lump clinically to come to a conclusion whether this is due to the fat, enlarged (hypertrophied) muscle or so. And to advise further investigations.
- MRI of the cervical spine and the soft tissue will give a proper diagnosis about the nature of the lump so that further treatment can be planned which may be surgery if this is a lipoma.
- Overweight definitely adds to the the already existing.
