Question: have migraines in the past. for several months they are different. Intense pressure in head, behind eyes, forehead.blurry vision.sudden sharp attacks with vomiting. meds don't touch them anymore. head movement, lying down can worsen/shift the pain. should i be seen by a Dr again? cannot function with these newest headaches

had recent optometric check-in. no pressure in eyes. vision still excellent, -2.0 glasses. temples very tender.

Brief Answer:
You need re-evaluation.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your symptoms and agree that you need to see the neurologist again.

This is because the nature of headache and accompanying symptoms have changed. It is still possible that they are related to migraines, however alternative diagnosis should be considered. These include ischemia (lack of blood supply to brain), venous sinus thrombosis, etc. This can be done by doing an MRI with MR venogram of brain.

I hope my reply has helped you.

I would be pleased to answer, if you have any follow up queries or if you require any further information.

Unfortunately the migraines still plague me.

Over the years I've seen neurologists and internists, I've been prescribed anti-migraine meds, keep highly hydrated with water, and try to get as much potassium & magnesium as I can.

The big question: WHY did my headaches change so drastically in the last 6 months. Their location and pain are very different from other years.

They have moved into my frontal lobe and temples (not previously), my eye balls and sockets throb (I've been seen by my Optometrist to rule out pressure or infection inside the eyes).

Other symptoms:

-I've had sharp stabbing pains that hit me out of nowhere like a lightning bolt (again, near the front of my head) This pain brings me to my knees!!

-seem to be worse in the morning, often wakening me in the night

-tenderness in the temples, sensation that my temples are going to explode.

-movement or leaning over doing dishes, vacuuming etc. worsen the headache tem-fold

-if I lay down I have the sensation that he pressure & blood pooling is in my head, and turns to concrete if I settle into a position. It feels like concrete when settled. I have to take extra sedative to help me sleep

-and the worst part is, no meds previously used (even morphine and its families) take it away.

Please, any suggestions?

Emerg turns me away full of medicinal cocktails that just make me foggy, but THE PAIN STAYS!

thank you,

Brief Answer:
Was the MRI done?

Detailed Answer:
Thank you for getting back.

I have noted your detailed symptoms. It is unfortunate to note that your pain and other symptoms have worsened, and moreover, you are not getting much help from the emergency doctors.

As suggested earlier, did you do the MRI of brain with venogram? This was necessary to exclude other conditions that can cause these pains and symptoms.

If the MRI and venogram are normal, then, these could be because of severe migraine. The underlying basis of these symptoms is related to changes in hormones and neurotransmitters in brain.

If medications do not work, then, botox injections are the best therapy for chronic severe migraine.

Best wishes,
