What Causes Mild Diarrhea During Sexual Intercourse?

Question: My partner and I (45yrs) have been dating a few months. (hetero-sexual) We feel very close and recently agreed to start a physical relationship. Together we had tests for HIV/STDs and happily received 'clear' results'. He has told me that he has some anxiety around sexual intimacy due to some kind of infection and emergency circumcision surgery when he was very young. (Not sure of age/details). He said that he is self-conscious due to scarring on his penis. Considering we are a relatively new couple, we are both open and communicate fairly well I think. I have done my best to reassure him. During our first sexual encounter, he initially had difficulty with erection but we relaxed and managed in the end. I noticed that he seemed to have mild diarrhea during sex which was a bit worrying to me. I didn't mention it to him as I sense that he is nervous enough without any extra stress. I would like to support him and pursue our relationship but I would like to have some understanding about what's going on. Do you have any idea what could be going on? He has talked with me about stress/breakdown in his life in the past and his anxiety around initial sexual intimacy due to performance and scarring (as mentioned above). I get the feeling that he is an anxious/worrier type person. (I am the opposite so maybe we can work.) If the problem was only erection, I would wait until our second physical encounter, but I don't get the diarrhea! Thanks for your help on this delicate issue.

I appreciate your positive attitude towards your relationship. In any long term relationship physical relationship always play a key role. You have clearly mentioned that your partner had past episodes of some anxiety problem as well as he has some still anxiety during intimacy.
During anxiety anal sphincters relax and stool may come out. So diarrhoea is a part of anxiety nothing else likely.
Positive point is you ended with success in first encounter.
In males performance anxiety plays an important role which may lead to problem in erection.
He can be counselled about this issue.


Sometime due to fatigue, lack of mood, situational problem or environmental changes man can fail in sex. It is normal phenomenon. However when this failure makes person apprehensive about erection, sex life is affected. This is called performance anxiety. After such failure when person actively thinks about erection or success of intercourse during sex his mind is likely diverted to such thoughts. For constant erection continuous exciting/romantic/erotic feelings should be there in mind. If mind diverted to concern about erection, erection would not be possible.

To get rid of performance anxiety help of partner is an important aspect.
Whenever you go for sex, spare enough time in foreplay. He should not think actively about erection, his mind must be occupied with sexual/romantic feelings. During foreplay you should try to arouse him and note that his penis gets erection or not. When you feel that penis is sufficiently erect you yourself should try to put erect penis in vagina gradually and he should continue foreplay feelings without being conscious about erection. He must try to enjoy and only enjoy. Once penis is able to enter in vagina and strokes starts it is easy to complete successful sex.
