What Causes Mild Fever, Cold And Cough When There Are TLC Fluctuations?

Question: I am male aged 53 years with 6 years diabetic type II history, previously HBA1C in the range of 9 now fasting and PP sugar levels in normal range, weight 130 kgs, previously hypertensive took medicines for BP for 20 years, now stopped BP medicines as BP being in the range 130/90, pulse rate in the range 90-110, taking Dytor 10, daily 1 tablet for fluid retention from many years, suffered from cold and cough in last week of november 2015, and from 4.12.15 had fever for 5 days in the range of 101, took antibiotic but low grade fever still persisted, was hospitalised, and detected with pneumonia, WBC 16000, CRP 11, and left lung basal haziness in chest x ray, was treated with antibiotic IV, steroid and nebulizer. In HRCT Thorax consolidation in left lower lobe. Now at home with no antibiotic and steroids. Please advice on the following:
1. Fluctuations in TLC in december 2015 on 8-16.4, 10-13.3, 13-17.0, 21-16.7, 26-14.1 and fluctuations in CRP in december 2015 on 10-11.33, 13-1.24, 21-2.89, 26-4.56?
2. Shall I go for bronchoscopy?
3. Shall I engage some lung specialist physiotherapist for excercise and massage?
4. Shall I use hot water bag compress and/or electric hot compress in the affected area?
5. Shall i practice home remedies like drinking hot milk and turmeric and apply cow ghee in the affected area?
6. What are the do's and dont's for me regarding my lifestyle, food and drinks intakes, medicines, walking, breathing excercise, use of AC, hot and cold intakes and others.

Fluctuations in TLC and CRP are mostly due to uncontrolled infection. Your infection is not fully controlled with drugs. So Fluctuations in TLC and CRP are seen.

Yes, you should definitely go for Bronchoscopy and XXXXXXX (bronchoalveolar lavage) analysis from the involved segment. Best treatment of pneumonia is appropriate antibiotic according to the isolated organism. XXXXXXX will isolate the causative organism and sensitivity report will guide you about appropriate effective antibiotic treatment.

Yes, chest physiotherapy and deep breathing exercises by specialist physiotherapist is advisable in consolidation.

If you are having pleuritic pain then warm water pad application will help otherwise no need for this.

Turmeric is having anti oxidant property and good in infection. So you should drink such things. No need of application of cow ghee on affected areas.

Drink 1.5 liters of food daily. Eat raw salads more. Your HbA1C is not controlled. So avoid sweet, ghee, oil and other fatty food. You should walk daily for 3-4 kms. And better to avoid AC till this pneumonia is over.

1. Which doctor will answer our question? Do we have the choice or the doctor on duty will answer?
2. How come when online the panel was showing Dr XXXXXXX , Dr XXXXXXX and Dr XXXXXXX in pulmonologist, Dr XXXXXXX answered the question?
3.In my 3 follow ups who will answer my questions Dr XXXXXXX or doctor on duty?
4. What shall I do if I also want an opinion from two other pulmonologist like Dr XXXXXXX or Dr Goyal?
5. What do you mean by premium questions and public questions?
6. I am also enclosing my temperature chart, sugar chart, BP pulse chart,my current medicines chart, discharge summary findings, HRCT thorax with contrast report and my current pulmonologists first prescription when my problem started of 25.11.15 for correlation and advice.
7. Can you advice which is the best place and doctor in XXXXXXX for bronchoscopy, XXXXXXX and physiotherapy?

Lastly I would like to know whether my treatment till now is in the right or wrong direction?

I'm XXXXXXX fortis hospital is having good Pulmonary medicine department. You can consult fortis hospital for Bronchoscopy, XXXXXXX and physiotherapy.
Or you can consult your primary pulmonologist for this. Almost all pulmonologist are good at doing Bronchoscopy and XXXXXXX
This is to isolate the causative organism. And also to rule out tuberculosis because tuberculosis is common in lower lobes in diabetic patients without classical symptoms.
And honestly speaking, no role of steroids in pneumonia patients. Because steroids actually lower down the immunity and can actually flare up infection. Steroids also rise blood sugar level and imbalance diabetes. So steroids given in your case is something not right.
