Brief Answer:
Please consult a cardiologist urgently and get complete heart evaluation

Detailed Answer:
Hi,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. After reading through your bloot test reports done on 23 October 2015, it is seen that you have high levels of triglycerides (736 mg/ dl) and total cholesterol (216 mg/ dl) is increased mildly. VLDL (147.18 mg/ dl) is increased.

2. The Hs CRP is showing a value of 4.35 which indicates high risk of coronary heart disease.

3. From the above findings and because you are having chest pain with associated sweating, it is suggested that you consult a cardiologist in XXXXXXX urgently and get complete heart evaluation done including ECG, echocardiography and detailed tests which might be recommended after clinical examination. In your case, possibility of coronary heart disease needs to be evaluated in detail.

You also have diabetes which is not controlled vitamin D deficiency.

Hope your query is answered.
Please do wirte back if you have any doubts.
