Black dots over the genitals can occur due to Naevi or moles or they could even be suggestive of warts .
But since there is no outside exposure in your case ,do the possibility of std is very less .
Still it is always good to get the suspicious areas checked .
See a nearby dermatologist as a skin specialist would be the best person to see in such problems .
